目的 探讨胸外科术后肺部感染应用痰热清注射液治疗的疗效及对血清可溶性白细胞分化抗原14(sCD14)、可溶性髓系细胞触发受体-1(sTREM-1)、C-反应蛋白(CRP)水平的影响.方法 选取2020年1月-2023年4月河北省中医院胸外科术后肺部感染患者120例,随机数字表法分为对照组(n=60)和研究组(n=60),对照组给予常规抗感染治疗,研究组在对照组治疗的基础上辅以痰热清注射液,均治疗2周.观察治疗2周后临床疗效及症状体征改善情况,分析治疗后患者肺功能指标、血清sCD14、sTREM-1、CRP水平变化及治疗期间的安全性.结果 与对照组总有效率(71.67％)比较,研究组治疗2周后的总有效率更高(88.33％)(P＜0.05);研究组咳嗽咳痰、肺部啰音、发热、胸片中炎性病灶消失时间短于对照组(P＜0.05);治疗2周后,研究组用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC升高幅度大于对照组(P＜0.05),血清sCD14、sTREM-1、CRP水平降低幅度大于对照组(P＜0.05);治疗期间,研究组和对照组均未出现胸闷、恶心、呕吐等不良反应.结论 痰热清注射液可有效调节胸外科术后肺部感染患者血清sCD14、sTREM-1、CRP水平,减轻机体炎症反应,并可改善患者肺功能,进而有利于改善患者症状体征,提高疗效,且安全性良好.
OBJECTIVE To explore the effect of Tanreqing injection on treatment of the thoracic surgery department patients with postoperative pulmonary infection and observe the influence on serum soluble leukocyte differentia-tion antigen 14(sCD14),soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and C-reactive pro-tein(CRP).METHODS A total of 120 patients who were treated in thoracic surgery department of Hebie Provin-cial Hospital of Traditional Chinese Medicine and had postoperative pulmonary infection from Jan 2020 to Apr 2023 were enrolled in the study and were randomly divided into the control group with 60 cases and the study group.The control group was given conventional anti-infection therapy,and the study group was given additional Tanreqing injection on basis of the treatment of the control group,both groups were treated for 2 weeks.The clin-ical therapeutic effects and improvement of symptoms and signs were observed after the treatment for 2 weeks,the lung function indexes,serum sCD14,sTREM-1 and CRP were observed and compared between the two groups of patients,and the safety during the treatment was analyzed.RESULTS The total effective rate of the study group was 88.33％after the treatment for 2 weeks,significantly higher than 71.67％of the control group(P＜0.05).The disappearance time of cough and expectoration,lung rales,fever and inflammatory lesions on chest CT were signifi-cantly shorter in the study group than in the control group(P＜0.05).The forced vital capacity(FVC),forced expirato-ry volume in the first second(FEV1)and FEV1/FVC of the study group rose more significantly than those of the control group(P＜0.05);the levels of serum sCD14,sTREM-1 and CRP of the study group declined more significantly than those of the control group(P＜0.05).There was no case of adverse reactions such as chest distress,nausea or emesis in the study group and the control group during the treatment.CONCLUSION Tanreqing injection may effectively regu-late the levels of serum sCD14,sTREM-1 and CRP of the thoracic surgery department patients with postoperative pulmonary infection,alleviate the inflammatory response and boost the lung function so as to improve the symp-toms and signs of the patients,with the curative effect favorable,the safety high.
