CYP1A2基因多态性与前列腺增生术后尿路感染的关联及MCP-1、SP-A/D、U-HBP和LEU的预测价值

目的 探讨CYP1A2基因多态性与前列腺增生术后尿路感染的关联及单核细胞趋化蛋白1(MCP-1)、表面活性蛋白(SP)-A/D、尿肝素结合蛋白(U-HBP)、白细胞酯酶(LEU)的预测价值.方法 选取268例邢台市人民医院2020年3月-2023年6月收治的前列腺增生患者,根据术后尿路感染情况分组,包括51例感染组和217例未感染组,统计术后尿路感染的病原菌分布;检测MCP-1、U-HBP、SP-A/D和LEU水平;受试者工作特征(ROC)曲线分析各指标对前列腺增生术后尿路感染的诊断价值.采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)检测两组患者CYP1A2基因多态性.结果 前列腺增生术后尿路感染发生率为19.03%;共分离病原菌63株,革兰阴性菌居多;感染组LEU阳性表达率及MCP-1、SP-A/D、U-HBP水平均高于未感染组(P<0.05);MCP-1、U-HBP、SP-A/D、LEU四者联合预测前列腺增生术后尿路感染的曲线下面积(AUC)为0.955;感染组CYP1A2 rs20695143859位点AA基因型频率和A等位基因频率较未感染组升高(P<0.05),GG基因型频率和G等位基因频率低于未感染组(P<0.05).结论 前列腺增生术后尿路感染发生率较高,其中革兰阴性菌占比较大;前列腺增生术后尿路感染MCP-1、SP-A/D、U-HBP呈高表达,LEU阳性表达率升高,联合检测有助于诊断前列腺增生术后尿路感染;CYP1A2基因多态性可能参与前列腺增生术后尿路感染病情进展.
Association between CYP1A2 gene polymorphism and postoperative urinary tract infection in benign prostatic hyperplasia patients and predictive values of MCP-1,SP-A/D,U-HBP and LEU
OBJECTIVE To explore the association between CYP1A2 gene polymorphism and postoperative urinary tract infection in the patients with benign prostatic hyperplasia(BPH)and analyze the predictive values of mono-cyte chemotactic protein 1(MCP-1),surfactant protein(SP)-A/D,uro-heparin-binding protein(U-HBP)and leukocyte esterase(LEU).METHODS A total of 268 patients with BPH who were treated in Xingtai People's Hospital from Mar 2020 to Jun 2023 were enrolled in the study and were divided into the infection group with 51 cases and the no infection group with 217 cases according to the status of postoperative urinary tract infection.The distribution of pathogens isolated from the patients with postoperative urinary tract infection was statistically ana-lyzed.The levels of MCP-1,U-HBP,SP-A/D and LEU were detected,the values of the indexes in diagnosis of the postoperative urinary tract infection were analyzed by means of receiver operating characteristic(ROC)curves,and the CYP1A2 gene polymorphisms were detected for the two groups of patients by polymerase-chain-reaction-restriction-fragment-length polymorphism(PCR-RFLP).RESULTS The incidence of postoperative urina-ry tract infection was 19.03%among the BPH patients.Totally 63 strains of pathogens were isolated,among which gram-negative bacteria were dominant.The positive rate of LEU and the levels of MCP-1,SP-A/D and U-HBP were significantly higher in the infection group than in the no infection group(P<0.05).The area under curve(AUC)of the joint detection of MCP-1,U-HBP,SP-A/D and LEU was 0.955 in prediction of postopera-tive urinary tract infection in the BPH patients.The frequencies of AA genotype and A allele at rs20695143859 lo-cus of CYP1A2 were significantly higher in the infection group than in the no infection group(P<0.05);the fre-quencies of GG genotype and G allele of the infection group were significantly lower than those of the no infection group(P<0.05).CONCLUSION The incidence of postoperative urinary tract infection is high among the BPH pa-tients,the gram-negative bacteria are dominant among the isolated pathogens.The BPH patients with postopera-tive urinary tract infection show high expressions of MCP-1,SP-A/D and U-HBP,the positive rate of expression of LEU is increased,and the joint detection of the above indexes may facilitate the diagnosis of postoperative uri-nary tract infection.CYP1A2 gene polymorphisms may be involved in the progression of illness condition of the BPH patients with postoperative urinary tract infection.
