摘要
目的 探讨补中益气汤加减治疗对老年肺部感染患者血清可溶性髓样细胞触发受体-1(sTREM-1)、高迁移率族蛋白B1(HMGB1)、干扰素-γ(IFN-γ)、降钙素原(PCT)水平的影响.方法 选择2020年4月-2023年4月衡水市中医医院收治的老年肺部感染患者98例随机纳入对照组(49例)和试验组(49例);对照组采用常规西医治疗,试验组在此基础上采用补中益气汤加减治疗,两组均治疗2周;比较两组治疗2周后的疗效,症状体征改善情况,治疗前和治疗2周后的中医症候、病情评分、血气及炎症指标水平.结果 治疗2周后,试验组疗效优于对照组(P＜0.001);与对照组比较,试验组症状和体征恢复正常时间更短(P＜0.05);治疗2周后与治疗前比较,两组中医症候、临床肺部感染评分(CPIS)、急性生理学和慢性健康状况评价Ⅱ(APACHE Ⅱ)评分、动脉二氧化碳分压(PaCO2)及血清sTREM-1、HMGB1、IFN-γ、PCT水平均降低,且试验组更低(P＜0.05);两组血氧饱和度(SaO2)、动脉血氧分压(PaO2)、氧合指数(OI)均升高,且试验组更高(P＜0.05).结论 补中益气汤加减治疗可调节老年肺部感染患者血清sTREM-1、HMGB1、IFN-γ、PCT水平,减轻机体炎症,并可改善患者中医症候及血气,进而可缓解患者病情,促进患者临床症状的改善,提高临床疗效.
Abstract
OBJECTIVE To investigate the effect of modified Buzhong Yiqi decoction on serum soluble myeloid cell triggering receptor-1(sTREM-1),high mobility group protein B1(HMGB1),infection-γ(IFN-γ),procalcitonin(PCT)levels in elderly patients with pulmonary infection.METHODS Totally 98 elderly patients with pulmonary infection admitted to Hengshui city hospital of traditional Chinese medicine from Apr.2020 to Apr.2023 were ran-domly divided into the control group(49 cases)and the experimental group(49 cases),and the group using a ran-dom number table method.The control group was treated with conventional Western medicine,while the experi-mental group was treated with Buzhong Yiqi decoction on this basis.Both groups were treated for 2 weeks.The ef-ficacy after 2 weeks of treatment,symptoms and signs improvement,traditional Chinese medicine symptoms,dis-ease scores,blood gas and inflammation index levels were compared between the two groups before and after 2 weeks of treatment.RESULTS After 2 weeks of treatment,the therapeutic effect of the experimental group was better than that of the control group(P＜0.001);compared with the control group,the recovery time of symp-toms and signs in the experimental group was shorter(P＜0.05),after 2 weeks of treatment compared with be-fore,the traditional Chinese medicine symptoms,clinical pulmonary infection score(CPIS),acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scores,arterial partial pressure of carbon dioxide(PaCO2),and serum sTREM-1,HMGB1,IFN-γ,and PCT levels in both decreased,experimental group was lower(P＜0.05),the blood saturation oxygen(SaO2),arterial partial pressure of oxygen(PaO2),and oxygenation index(OI)in-creased,experimental group was higher(P＜0.05).CONCLUSION Buzhong Yiqi decoction could regulate the elderly patients with pulmonary infection serum sTREM-1,HMGB1,IFN-y,PCT levels,reduce the body inflam-mation,improve symptoms of traditional Chinese medicine symptoms and blood gas,then alleviate the patient's condition,promote the improvement of clinical symptoms and improve clinical efficacy.
