Application of ICU bedside ultrasound in assessment of cardiac function in elderly patients with sepsis and impact on prognosis
OBJECTIVE To analyze the application of bedside ultrasound in intensive care unit(ICU)in the assess-ment of cardiac function of elderly patients with sepsis,and its relationship with the prognosis.METHODS A total of 108 elderly patients with sepsis admitted to ICU of Chengbei District Hangzhou First People's Hospital between Apr.2020 and Apr.2023 were selected and were divided into the ultrasound group and the control group by ran-dom number expression method,with 54 cases in each group.Fluid resuscitation was carried out in the control group by the pulse-indicator continuous cardiac output monitoring,while fluid resuscitation was carried out in the ultrasound group by bedside ultrasound monitoring,and the clinical data such as gender,age and body mass index(BMI)were recorded.Heart rate(HR),central venous pressure(CVP),mean arterial pressure(MAP)and cen-tral venous oxygen saturation(ScvO2)in the control group and the ultrasound group before and after 12 h of treat-ment were monitored.The rates of fluid resuscitation target achievement and fluid resuscitation dosage after 6 h and 12 h of treatment in both groups were statistically analyzed.The cardiac function indicators after 12h of treat-ment were compared between the two groups.The duration of mechanical ventilation,length of ICU hospitaliza-tion,and survival status at 28-day of admission were recorded.RESULTS After 12 h of treatment,MAP,CVP and ScvO2 in both groups increased,and the MAP,CVP and ScvO2 in the ultrasound group were higher than those in the control group(P<0.05).There was no statistically significant difference in HR of both groups before treatment and after 12 h of treatment.After 6 h and 12 h of treatment,the rate of fluid resuscitation target a-chievement in the ultrasound group was higher than that in the control group,while the liquid resuscitation dosage was lower than that in the control group(P<0.05).Cardiac output(CO)and Ejection fraction(EF)in the ultra-sound group were higher than those in the control group,while E/e'was lower than that in the control group(P<0.05).There was no statistically significant difference in mechanical ventilation time,length of ICU hospital-ization,and survival rate at 28-day between the two groups.CONCLUSION The application of ICU bedside ultra-sound in liquid resuscitation of elderly patients with sepsis could improve their cardiac function and hemodynamic indicators,increase the rate of fluid resuscitation target achievement,and reduce the liquid resuscitation dosage.
