Variation of drug resistance of representative species of Enterobacteriaceae in a three-A hospital from 2013 to 2022 and distribution features
OBJECTIVE To explore the change of drug resistance of representative species of Enterobacteriaceae in a three-A hospital from 2013 to 2022 and observe the distribution features.METHODS The data of isolation of En-terobacteriaceae from various submitted clinical specimens were collected from 3201 Hospital from Jan 2013 to Dec 2022.The distribution and drug resistance rates of the strains were statistically analyzed by WHONET 5.6 soft-ware,the isolation rates of species and drug resistance rates were compared among the years by using SPSS 27.0 software.The changes of drug resistance trends and clinical distribution of the Enterobacteriaceae Escherichia co-li,Klebsiella pneumoniae and Enterobacter cloacae were retrospectively reviewed.RESULTS Totally 26 535 strains of pathogens were isolated from 2013 to 2022,5 107(19.25%)of which were E.coli,2 398(9.04%)were K.pneumoniae,and 609(2.30%)were E.cloacae.The isolation rates of E.coli and K.pneumoniae showed up-ward trends,while the isolation rate of E.cloacae showed downward trend(P<0.05).The drug resistance rate of E.coli strains to cefotaxime was more than 50%,showing a downward trend(P<0.05),the drug resistance rates to imipenem and meropenem were no more than 1%,showing upward trends(P<0.05).The drug resistance rates of the K.pneumoniae strains to cephalosporins showed downward trends in varying degrees(P<0.05).The drug resistance rates of the E.cloacae strains to cephalosporins were more than 30%,and the drug resistance rates to imipenem and meropenem were increased from 1.1%in 2013 to 23.4%in 2022(P<0.05).The E.coli,K.pneu-moniae and E.cloacae strains were mainly isolated from surgical departments,and the isolation rates showed up-ward trends(P<0.05).The E.coli strains were mainly isolated from urine specimens,the K.pneumoniae and E.cloacae strains were mainly isolated from respiratory tract specimens,but the isolation rates showed downward trends(P<0.05).CONCLUSION The isolation rates of Enterobacteriaceae are increased year by year,the drug re-sistance rates to some types of antibiotics show downward trends,but the drug resistance rates to carbapenems show upward trends.
