Expressions of miR-21,miR-19b,IL-37 and PCT in peripheral blood of acute myocardial infarction patients complicated with nosocomial infection and values in prediction of prognosis
OBJECTIVE To explore the expressions of microribonucleic acid(miR)-21,miR-19b,interleukin(IL)-37 and procalcitonin(PCT)in peripheral blood of the acute myocardial infarction(AMI)patients complicated with nosocomial infection and analyze the values in prediction of prognosis.METHODS A total of 108 AMI patients who were complicated with nosocomial infection and were treated in People's Hospital Affiliated to Shandong First Medical University from Jul 2020 to Jul 2023 were assigned as the infection group,meanwhile,111 AMI patients who did not have nosocomial infection were chosen as the no infection group.The patients of the infection group were divided into the favorable prognosis group with 82 cases and the poor prognosis group with 26 cases according to the 3-month prognosis.The distribution of pathogens isolated from the infection group was statistically ana-lyzed,the levels of serum miR-21,miR-19b,IL-37 and PCT were compared among the infection group,the no infection group,the favorable prognosis group and the poor prognosis group.The values of the four indexes in pre-diction of adverse prognosis of the AMI patients complicated with nosocomial infection were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS Totally 123 strains of pathogens were isolated from the infection group,75(60.98%)of which were gram-negative bacteria,and Klebsiella pneumoniae was the predom-inant species of gram-negative bacteria.The levels of serum miR-21,miR-19b,IL-37 and PCT of the infection group were significantly higher than those of the no infection group(P<0.05).The levels of serum miR-21,miR-19b,IL-37 and PCT of the poor prognosis group were significantly higher than those of the favorable prognosis group(P<0.05).The area under curve(AUC)of the joint detection of miR-21,miR-19b,IL-37 and PCT was significantly higher than that of the single detection in diagnosis of the poor prognosis of the AMI patients compli-cated with nosocomial infection(P<0.05),and the sensitivity of the joint detection was 80.77%,with the speci-ficity 91.46%.CONCLUSION The gram-negative bacteria are dominant among the pathogens isolated from the AMI patients complicated with nosocomial infection.The patients show the high expressions of serum miR-21,miR-19b,IL-37 and PCT,and the joint detection of the four indexes has high value in prediction of the poor prognosis.
