Endoscopic features,expressions of miR-1296-5p,miR-155-5p and Runx3 in patients with chronic atrophic gastritis and their association with Hp infection

目的 分析慢性萎缩性胃炎(CAG)患者内镜特征、胃黏膜组织微小核糖核酸(miR)-1296-5p、miR-155-5p、Runx3表达情况及与幽门螺杆菌(Hp)感染的关系.方法 选择达州市中西医结合(第二人民)医院2021年5月-2023年5月收治的CAG患者151例为研究对象.根据胃黏膜萎缩程度分为轻症组75例、中症组51例和重症组25例;比较不同胃黏膜萎缩程度患者的内镜特征、miR-1296-5p、miR-155-5p、Runx3表达及Hp感染情况;采用Spearman分析miR-1296-5p、miR-155-5p、Runx3表达与Hp感染的相关性;比较CAG合并Hp不同预后患者miR-1296-5p、miR-155-5p、Runx3表达情况;采用受试者工作特征(ROC)曲线分析miR-1296-5p、miR-155-5p、Runx3对CAG合并Hp感染患者不良预后的诊断价值.结果 轻症组各内镜特征占比低于中、重症组(P＜0.05);重症组miR-1296-5p、miR-155-5p及Runx3表达低于中、轻症组,中症组低于轻症组(P＜0.05);重症组 Hp 阳性率高于轻症组(P＜0.05);miR-1296-5p、miR-155-5p、Runx3 与 Hp 感染呈负相关(P＜0.05);CAG 合并Hp感染预后不良组miR-1296-5p、miR-155-5p及Runx3表达低于预后良好组(P＜0.05);miR-1296-5p、miR-155-5p、Runx3联合诊断CAG合并Hp感染患者不良预后的曲线下面积(AUC)高于单一检测(P＜0.05).结论 CAG患者内镜特征与胃黏膜萎缩程度有关,miR-1296-5p、miR-155-5p、Runx3表达与Hp感染呈负相关,且三者联合检测可提高对CAG合并Hp感染患者不良预后的诊断价值.
OBJECTIVE To observe the endoscopic characteristics,expressions of microribonucleic acid(miR)-1296-5p,miR-155-5p and Runx3 in gastric mucosa tissues of the patients with chronic atrophic gastritis(CAG)and analyze the association with Helicobacter pylori(Hp)infection.METHODS Totally 151 patients with CAG who were treated in Dazhou Integrated Traditional Chinese and Western Medicine(Second People's Hospital)from May 2021 to May 2023 were recruited as the research subjects and were divided into the mild group with 75 cases,the moderate group with 51 cases and the severe group with 25 cases according to the severity of gastric mucosa atrophy.The endoscopic features,expressions of miR-1296-5p,miR-155-5p and Runx3,and prevalence of Hp infection were compared among the patients with different severities of gastric mucosa atrophy.Spearman a-nalysis was performed for the association between the expressions of miR-1296-5p,miR-155-5p and Runx3 and the Hp infection.The expressions of miR-1296-5p,miR-155-5p and Runx3 of the CAG patients complicated with Hp infection were compared between the favorable prognosis group and the poor prognosis group.The values of miR-1296-5p,miR-155-5p and Runx3 in diagnosis of poor prognosis were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The percentage of patients with endoscopic characteristics was lower in the mild group than in the moderate group and the severe group(P＜0.05).The expression levels of miR-1296-5p,miR-155-5p and Runx3 of the severe group were lower than those of the moderate group and the mild group,and the expression levels of the above indexes of the moderate group were lower than those of the mild group(P＜0.05).The positive rate of Hp of the severe group was higher than that of the mild group(P＜0.05).The miR-1296-5p,miR-155-5p and Runx3 were negatively correlated with the Hp infection(P＜0.05).The expression lev-els of miR-1296-5p,miR-155-5p and Runx3 of the CAG patients complicated with Hp infection were lower in the poor prognosis group than in the favorable prognosis group(P＜0.05).The area under curve(AUC)of the joint detection of miR-1296-5p,miR-155-5p and Runx3 was higher than that of the single detection in diagnosis of the poor prognosis of the CAG patients complicated with Hp infection(P＜0.05).CONCLUSION The endoscopic fea-tures of the CAG patients are associated with the degree of gastric mucosa atrophy,the expressions of miR-1296-5p,miR-155-5p and Runx3 are negatively correlated with the Hp infection,and the joint detection of the three in-dexes can raise the value in diagnosis of the poor prognosis of the CAG patients complicated with Hp infection.
