Pathogenic bacteria isolated from primary liver cancer patients with infection after transcatheter arterial chemoembolization intervention and predictive values of serum miR-200a,miR-221,IL-8 and CRP
OBJECTIVE To explore the pathogenic bacteria isolated from the primary liver cancer patients with in-fection after transcranial arterial chemoembolization(TACE)intervention and analyze the predictive values of ser-um microribonucleic acid(miR)-200a,miR-221,interleukin(IL)-8 and C-reactive protein(CRP).METHODS To-tally 49 primary liver cancer patients who underwent TACE and had postoperative infection in Shengjing Hospital of China Medical University from Aug.1 2021 to Aug.1 2023 were assigned as the infection group,meanwhile,51 primary liver cancer patients who underwent TACE but did not have postoperative infection were chosen as the non-infection group.The infection sites of the patients in the infection group and distribution of pathogens were statistically analyzed.The levels of serum miR-200a,miR-221,IL-8 and CRP were compared between the two groups,and the values of miR-200a,miR-221,IL-8 and CRP in prediction of postoperative infection in the prima-ry liver cancer patients undergoing TACE were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS Among the 49 patients of the infection group,the patients with organ/lacunar infection accoun-ted for 77.55%,and the patients with other infections accounted for 22.45%.Totally 57 strains of pathogens were isolated,among which gram-negative bacteria were dominant,followed by gram-positive bacteria.Pseudomonas aeruginosa and Klebsiella pneumoniae were the predominant species of the gram-negative bacteria;Staphylococ-cus aureus was the predominant species of the gram-positive bacteria.The levels of serum miR-221,IL-8 and CRP of the infection group were significantly higher than those of the non-infection group,while the serum miR-200a of the infection group was significantly lower than that of the non-infection group(P<0.05).The area under curve(AUC)of the joint detection of miR-200a,miR-221,IL-8 and CRP was significantly higher than that of the single detection in diagnosis of the postoperative infection in the primary liver cancer patients undergoing TACE(P<0.05),and the sensitivity of the joint detection was 97.96%,with the specificity 82.35%.CONCLUSION The or-gan/lacuna is the major infection site of the primary liver cancer and TACE patients with postoperative infection.The gram-negative bacteria are dominant among the pathogens,the patients show the abnormal expressions of se-rum miR-200a,miR-221,IL-8 and CRP;the joint detection of the four indexes has high value in prediction of the postoperative infection in the primary liver cancer patients undergoing TACE.
