Relationship between postoperative abdominal infection and peripheral blood miR-21-3p,miR-22-3p and Ghrelin of patients undergoing pancreaticoduodenectomy and diagnostic values
OBJECTIVE To explore the association between the postoperative abdominal infection and the peripheral blood microribonucleic acid(miR)-21-3p,miR-22-3p,and growth hormone releasing peptide(Ghrelin)of the pa-tients undergoing pancreaticoduodenectomy(PD)and analyze the diagnostic values.METHODS A total of 51 pa-tients who underwent PD and had postoperative abdominal infection in Huanggang Central Hospital from Jun 2021 to Jun 2023 were randomly enrolled in the study and assigned as the study group,52 patients who underwent PD but did not have postoperative abdominal infection were chosen as the control group.The distribution of pathogens was statistically analyzed,the levels of peripheral blood miR-21-3p,miR-22-3p and serum Ghrelin were compared between the two groups,and the association with the postoperative abdominal infection was analyzed,and the val-ues of the single and joint detection of the indexes in diagnosis of the postoperative abdominal infection were ob-served.RESULTS Totally 51 strains of pathogens were isolated from the 51 PD patients with postoperative abdom-inal infection.Klebsiella pneumoniae and Escherichia coli were dominant among gram-negative bacteria;Entero-coccus faecalis and Enterococcus faecium were the predominant species of gram-positive bacteria.The levels of pe-ripheral blood miR-21-3p,miR-22-3p and serum Ghrelin of the study group were higher than those of the control group(P<0.05).The peripheral blood miR-21-3p,miR-22-3p and serum Ghrelin were positively correlated with the postoperative abdominal infection(P<0.05).The area under curve(AUC)of the joint detection of peripheral blood miR-21-3p,miR-22-3p and serum Ghrelin was higher than that of the single detection in diagnosis of the postoperative abdominal infection in the PD patients(P<0.05),and the sensitivity of the joint detection was 90.20%,with the specificity 94.23%.CONCLUSION K.pneumoniae,E.coli,E.faecalis and E.faecium are dominant among the pathogens isolated from the PD patients with postoperative abdominal infection.The PD pa-tients with postoperative abdominal infection show the high expressions of peripheral blood miR-21-3p,miR-22-3p and serum Ghrelin,which are associated with the postoperative abdominal infection.The joint detection of the three indexes has high value in diagnosis of the postoperative abdominal infection in the PD patients.
