直肠癌术后结肠造口感染血清miRNAs、CD36 mRNA、mTORC1 mRNA表达及与PCT的相关性

目的 探讨直肠癌根治术后结肠造口旁软组织感染患者微小核糖核酸(miR)-15a、miR-29b、分化抗原36(CD36)mRNA、哺乳动物雷帕霉素靶蛋白复合物1(mTORC1)mRNA表达及与降钙素原(PCT)的相关性.方法 选取2020年8月-2023年7月济宁医学院附属医院收治的91例直肠癌根治术结肠造口患者,根据结肠造口术后7 d内是否发生结肠造口旁软组织感染分为感染组27例和未感染组64例.分析两组肠道菌群失调分度;比较两组血清miR-15a、miR-29b、CD36 mRNA、mTORC1 mRNA、PCT水平;受试者工作特征(ROC)曲线分析联合检测的预测价值.Pearson法分析miR-15a、miR-29b、CD36 mRNA、mTORC1 mRNA与PCT水平相关性.结果 直肠癌根治术后结肠造口旁软组织感染发生率为29.67％.感染组Ⅲ度菌群失调比例、血清miR-29b、CD36 mRNA、mTORC1 mRNA、PCT水平比未感染组高(P＜0.05),miR-15a水平比未感染组低(P＜0.05),联合检测直肠癌根治术后结肠造口旁软组织感染的曲线下面积(AUC)比单独检测高(P＜0.05);miR-29b、CD36 mRNA、mTORC1水平与PCT呈正相关(P＜0.05);miR-15a水平与PCT呈负相关(P＜0.05).结论 直肠癌根治术后结肠造口旁软组织感染发生率高,存在肠道菌群失衡,血清miR-29b、CD36 mRNA、mTORC1 mRNA呈高表达,miR-15a呈低表达,联合检测可预测直肠癌根治术后结肠造口旁软组织感染,直肠癌根治术后结肠造口旁软组织感染患者miR-15a、miR-29b、CD36 mRNA、mTORC1 mRNA表达水平与PCT水平有关.
Expressions of serum miRNAs,CD36 mRNA,MTORC1 mRNA in rectal cancer patients with postoperative soft tissue infection beside colostomy and association with PCT
OBJECTIVE To explore the expressions of micro ribonucleic acid(miR)-15a,miR-29b,differentiation antigen 36(CD36)mRNA and mammalian target of rapamycin complex 1(mTORC1)mRNA in the rectal cancer patients with soft tissue infection beside colostomy after radical resection and analyze the association with procalci-tonin(PCT).METHODS Totally 91 rectal cancer patients with colostomy who underwent radical resection in Af-filiated Hospital of Jining Medical College from Aug 2020 to Jul 2023 were enrolled in the study and were divided into the infection group with 27 cases and the no infection group with 64 cases according to the status of para-co-lostomy soft tissue infection within 7 days after the surgery.The scale of intestinal flora imbalance was observed,the expression levels of serum miR-15a,miR-29b,CD36 mRNA,mTORC1 mRNA and PCT were compared be-tween the two groups.The predictive value of the joint detection of the indexes was analyzed by means of receiver operating characteristic(ROC)curves.Pearson analysis was performed for the association between miR-15a,miR-29b,CD36 mRNA,mTORC1 mRNA and PCT.RESULTS The incidence of soft tissue infection beside colostomy was 29.67％among the rectal cancer patients after radical resection.The proportion of patients with degree Ⅲ flo-ra imbalance and the levels of serum miR-29b,CD36 mRNA,mTORC1 mRNA and PCT were significantly higher in the infection group than in the no infection group(P＜0.05),while the miR-15a level of the infection group was significantly lower than that of the no infection group(P＜0.05).The area under curve(AUC)of the joint detec-tion of the indexes was significantly higher than that of the single detection in prediction of the soft tissue infection beside the colostomy(P＜0.05).The levels of miR-29b,CD36 mRNA and mTORC1 were positively correlated with PCT(P＜0.05),while the miR-15a level was negatively correlated with the PCT(P＜0.05).CONCLUSION The incidence of soft tissue infection beside colostomy is high among the rectal cancer patients after radical resec-tion.The patients have imbalance of intestinal flora and show high expressions of serum miR-29b,CD36 mRNA,mTORC1 mRNA and low expression of miR-15a.The joint detection of the indexes can predict the soft tissue in-fection.The expressions of miR-15a,miR-29b,CD36 mRNA and mTORC1 mRNA in the rectal cancer patients with soft tissue infection beside colostomy after radical resection are associated with PCT.
