宫颈HPV持续感染临床特征及宫颈脱落细胞中miR-148a-3p、Pax 1、LMX 1A 表达的诊断价值

目的 探究宫颈人乳头瘤病毒(HPV)持续感染临床特征及脱落细胞中微小RNA-148a-3p(miR-148a-3p)、配对盒基因1(Pax1)、LIM同源盒转录因子1α(LMX 1A)表达的诊断价值.方法 选取中卫市中医医院2019年12月-2022年12月收治的289例宫颈HPV阳性患者,随访6个月,根据是否发生HPV持续感染分为HPV持续感染组107例和HPV非持续感染组182例.分析宫颈HPV阳性患者HPV持续感染发生的临床特征,比较两组脱落细胞中miR-148a-3p、Pax1、LMX 1A表达水平,采用受试者工作特征(ROC)曲线分析其对宫颈HPV持续感染的诊断价值.结果 289例宫颈HPV阳性患者HPV持续感染发生107例,HPV持续感染发生率为37.02%.两组生殖道炎症、人流次数比较差异有统计学意义(P<0.05);HPV持续感染组脱落细胞中miR-148a-3p、Pax 1、LMX 1A表达水平低于HPV非持续感染组(P<0.05).脱落细胞中miR-148a-3p、Pax 1、LMX 1A联合检测诊断宫颈HPV阳性患者HPV持续感染的曲线下面积(AUC)高于各指标单一检测(P<0.05),且联合检测敏感度、特异度为88.79%、80.22%.结论 宫颈HPV阳性患者HPV持续感染发生率高,其脱落细胞中miR-148a-3p、Pax1、LMX1A水平呈低表达,且三者联合检测可提高宫颈HPV阳性患者HPV持续感染的诊断价值.
Clinical characteristics of patients with persistent cervical HPV infection and diagnostic values of miR-148a-3p,Pax 1 and LMX 1A in cervical exfoliated cells
OBJECTIVE To explore the clinical characteristics of the patients with persistent cervical human papillo-mavirus(HPV)infection and analyze the diagnostic values of micro ribonucleic acid-148a-3p(miR-148a-3p),paired box gene 1(Pax 1)and LIM homeobox transcription factor 1α(LMX 1A)in exfoliated cells.METHODS Totally 289 cervical HPV-positive patients who were treated in Zhongwei Traditional Chinese Medicine Hospital from Dec 2019 to Dec 2022 were enrolled in the study and were followed up for 6 months,the enrolled patients were divided into the persistent HPV infection group with 107 cases and the non-persistent HPV infection group with 182 cases according to the status of persistent HPV infection.The clinical characteristics of the HPV-positive patients with persistent HPV infection were analyzed,the expression levels of miR-148a-3p,Pax 1 and LMX 1A were compared between the two groups,and the values of the indexes in diagnosis of persistent cervical HPV in-fection were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS Among the 289 cervi-cal HPV-positive patients,107 had persistent HPV infection,with the incidence rate 37.02%.There were signifi-cant differences in the genital tract inflammations and number of times of induced abortion between the two groups(P<0.05).The expression levels of miR-148a-3p,Pax 1 and LMX 1A in the exfoliated cells were significantly lower in the persistent HPV infection group than in the non-persistent HPV infection group(P<0.05).The area under curve(AUC)of the joint detection of miR-148a-3p,Pax 1 an LMX 1A in exfoliated cells was significantly higher than that of the single detection(P<0.05),and the sensitivity of the joint detection was 88.79%,with the specificity 80.22%.CONCLUSION The incidence of persistent HPV infection is high among the cervical HPV-pos-itive patients,and the patients show low expression levels of miR-148a-3p,Pax 1 and LMX 1A in exfoliated cells.The joint detection of the three indexes can raise the diagnostic value of persistent HPV infection in the cervical HPV-positive patients.
