摘要
目的 探讨NOD样受体蛋白3(NLRP3)/凋亡相关斑点样蛋白(ASC)/半胱氨酸蛋白酶1(Caspase-1)信号通路在儿童腺病毒肺炎引发心肌应激损伤中的作用.方法 选取2021年1月-2023年1月于南阳医学高等专科学校第一附属医院就诊的腺病毒肺炎患儿112例为研究对象,根据有无心肌损伤分为无心肌损伤组76例、心肌损伤组36例,检测并比较两组病毒滴度、心肌酶谱水平、炎性因子、氧化应激因子、NLRP3通路相关mRNA、蛋白表达水平.结果 病毒载量为4×100~6×100 copy/μg的患儿占比最高,无心肌损伤组和心肌损伤组分别为33例(43.42%)、28例(45.16%);与无心肌损伤组比较,心肌损伤组肌钙蛋白Ⅰ(cTnI)、肌红蛋白(Mb)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸盐脱氢酶(LDH)、门冬氨酸氨基转移酶(AST)表达水平高,白细胞(WBC)计数、超敏C-反应蛋白(hs-CRP)、降钙素原(PCT)、白细胞介素(IL)-1β、IL-18、转化生长因子β(TGF-β)水平高,丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肤过氧化物酶(GSH-Px)水平高(P<0.05);NLRP3、ACS、Caspase-1的mRNA转录水平及蛋白相对表达水平高(P<0.05).结论 血清NLRP3、ACS、Caspase-1水平与腺病毒肺炎导致的心肌损伤相关指标相关,腺病毒可能是通过NLRP3/ACS/Caspase-1信号通路的激活介导氧化应激水平,参与病理损伤进程.
Abstract
OBJECTIVE To explore the role of NOD-like receptor protein 3(NLRP3)/apoptosis-associated speck-like protein containing a CARD(ASC)/cysteinyl aspartate specific proteinase 1(Caspase-1)in myocardial stress injury induced by adenovirus pneumonia in the children.METHODS A total of children with adenovirus pneumonia who were treated in the First Affiliated Hospital of Nanyang Medical College from Jan 2021 to Jan 2023 were re-cruited as the research subjects and were divided into the no myocardial injury group with 76 cases and the myocar-dial injury group with 36 cases according to the status of myocardial injury.The virus titer,myocardial enzyme level,and expression levels of inflammatory factors,oxidative stress factors,NLRP3 pathway-related mRNA and proteins were detected and compared between the two groups.RESULTS The proportion of the children with viral load ranging between 4×100 and 6×100 copy/µg was the highest,and there were 33(43.42%)cases in the no myocardial injury group and 28(45.16%)cases in the myocardial injury group.The expression levels of cardiac troponin Ⅰ(cTnⅠ),myohemoglobin(Mb),creatine kinase(CK),creatine kinase-isozyme(CK-MB),lactate de-hydrogenase(LDH)and aspartate aminotransferase(AST)of the myocardial injury group were significantly high-er than those of the no myocardial injury group,the levels of white blood cell(WBC),high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),interleukin(IL)-1β,IL-18 and transforming growth factor β(TGF-β)of the myocardial injury group were significantly higher than those of the no myocardial injury group;the levels of malondialdehyde(MDA),superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)of the myocardial injury group were higher than those of the no myocardial injury group(P<0.05).The mRNA transcription levels of NLRP3,ACS and Caspase-1 and the relative expression levels of proteins were significantly higher(P<0.05).CONCLUSION The levels of serum NLRP3,ACS and Caspase-1 are associated with the related factors for the my-ocardial injury induced by adenovirus pneumonia,and the adenovirus may mediate the oxidative stress level by ac-tivating NLRP3/ACS/Caspase-1 signaling pathways and participate in the process of pathological injury.
