目的 探讨原发性肺癌放疗后肺部感染患者微小核糖核酸(miR)-129、miR-146a、长链非编码RNA(ln-cRNA)核富含丰富的转录本(NEATI)及与辅助性T细胞(Th)1/Th2细胞因子水平的关系。方法 选取2019年7月一2023年7月南阳医学高等专科学校第一附属医院收治的原发性肺癌放疗患者109例,根据合并肺部感染情况分为感染组/未感染组(28例/81例);比较两组血清miR-129、lncRNA NEAT1、白细胞介素(IL)-2、干扰素-γ(IFN-γ)、IL-4、IFN-γ/IL-4及外周血miR-146a水平;通过受试者工作特征(ROC)曲线分析其单独及联合检测对原发性肺癌放疗后肺部感染预测价值;采用Pearson法分析原发性肺癌放疗后肺部感染患者miR-129、miR-146a、lncRNA NEAT1水平与Th1/Th2细胞因子水平的相关性。结果 感染组血清lncRNA NEAT1、IL-4和外周血miR-146a水平较未感染组高(P<0。05),血清miR-129、IL-2、IFN-γ、IFN-γ/IL-4水平较未感染组低(P<0。05);绘制ROC曲线获得miR-129、miR-146a、lncRNA NEAT1联合检测对原发性肺癌放疗后肺部感染的曲线下面积(AUC)高于各项单独检测(P<0。05),敏感度和特异度高;Pearson相关性分析结果发现IL-2、IFN-γ/IL-4与miR-146a、lncRNA NEAT1呈负相关(P<0。05),与miR-129呈正相关(P<0。05)。结论 原发性肺癌放疗后发生肺部感染miR-129、miR-146a、lncRNA NEAT1水平表达异常,三者联合检测对其诊断价值较高,且miR-129、miR-146a、lncRNA NEAT1 表达水平与 Th1/Th2 因子有关。
Expression levels of miR-129,miR-146a and lncRNA NEAT1 and their relationship with Th1-and Th2-related cytokines in patients with pulmonary infection after radiotherapy for primary lung cancer
OBJECTIVE To investigate the variation levels of the expression levels of micronucleic acid(Mir-129),miR-146a and long non-coding RNA(lncRNA)nuclear rich transcript(NEATI)in patients with pulmonary infec-tion after radiotherapy for primary lung cancer and their relationship with the levels of helper T cell(Th)1-and Th2-related cytokines.METHODS A total of 109 patients with primary lung cancer treated by radiotherapy in the First Affiliated Hospital of Nanyang Medical College from Jul 2019 to Jul 2023 were recruited and divided into the infected group and uninfected group(28 cases and 81 cases,respectively)according to whether the pulmonary in-fection occurred.The levels of serum miR-129,lncRNA NEAT1,interleukin(IL)-2,interferon-γ(IFN-γ),IL-4,IFN-γ/IL-4 and peripheral blood miR-146a were detected and compared between the two groups.The value of sin-gle and combined detection in predicting lung infection after radiotherapy for primary lung cancer was analyzed by receiver operating characteristic(ROC)curve.Pearson method was used to analyze the correlation of the levels of miR-129,miR-146a and lncRNA NEAT1 with Th1 and Th2 cytokines in patients with pulmonary infection after radiotherapy for primary lung cancer.RESULTS The levels of serum lncRNA NEAT1,IL-4 and peripheral blood miR-146a in the infected group were higher than those in the uninfected group(P<0.05),while the levels of ser-um miR-129,IL-2,IFN-γ and IFN-γ/IL-4 were lower than those in the uninfected group(P<0.05).The area under the curve(AUC)of combined detection of miR-129,miR-146a and lncRNA NEAT1 for the diagnosis of pulmonary infection after radiotherapy for primary lung cancer was higher than that of single detection(P<0.05),with higher sensitivity and specificity.Pearson correlation analysis showed that levels of miR-146a and ln-cRNA NEAT1 were negatively correlated with IL-2 and IFN-γ/IL-4(P<0.05)while was positively correlated with the level of miR-129(P<0.05).CONCLUSION Abnormal expression levels of miR-129,miR-146a and ln-cRNA NEAT1 were detected in patients with lung infection after radiotherapy in primary lung cancer.The com-bined detection of these three indexes is of high diagnostic value,and the expression levels of miR-129,miR-146a and lncRNA NEAT1 are related to Th1-and Th2-related cytokines.
