Value of joint dection of PCT,HNL and SAA in diagnosis of postoperative infection in osteosarcoma patients and risk factors
OBJECTIVE To explore the predictive value of preoperative levels of procalcitonin(PCT),human neu-trophil apolipoprotein(HNL)and serum amyloid A(SAA)and the risk factors of postoperative incision infection in patients with osteosarcoma.METHODS A total of 589 patients with osteosarcoma treated in the People's Hospi-tal of Hai'an from Jan 2021 to Jan 2023 were included in the study.They were divided into the infected group(n=52)and uninfected group(n=537)according to whether postoperative incision infection occurred.The incidence of postoperative incision infection in patients with osteosarcoma was analyzed.Multivariate logistic regression anal-ysis was used to analyze the risk factors of postoperative incision infection in osteosarcoma patients,and the pre-operative PCT.HNL and SAA levels were compared between the two groups.Receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of PCT,HNL and SAA alone and combined in postopera-tive incision infection in patients with osteosarcoma.RESULTS Postoperative incision infection occurred in 52 of 589 patients with osteosarcoma,and the incidence was 8.83%.Radiotherapy,low white blood cells,long opera-tion time,and low neutrophils were risk factors for postoperative incision infection in patients with osteosarcoma(P<0.05).Compared with the uninfected group,the expression levels of PCT,HNL and SAA in the infected group increased(P<0.05).The AUC value of the combined detection for the diagnosis of postoperative incision infection in osteosarcoma patients was higher than that of the single detection(P<0.05);the sensitivity and spe-cificity of the combined detection were 96.15%and 80.63%,respectively,which showed high diagnostic value.CONCLUSION Patients with osteosarcoma have a higher risk of postoperative incision infection,which is related to the increased levels of PCT,HNL and SAA before surgery,and its risk factors include receiving radiotherapy,low leukocytes,long operation time,and low neutrophils.The combined detection of PCT,HNL and SAA is of high value in the diagnosis of postoperative incision infection in patients with osteosarcoma.
