Expressions of TLR4,NF-κB and TRAF6 proteins in placenta of preterm premature rupture of fetal membrane patients complicated with chorioamnionitis and matenal and infant outcomes
OBJECTIVE To investigate the correlation of the protein expression of toll-like receptor 4(TLR4),nu-clear transcription factor-κB(NF-κB)and tumor necrosis factor receptor-related factor 6(TRAF6)in placenta with the occurrence,the stage of chorioamniotic infection and maternal and infant outcomes in patients with pre-term premature rupture of fetal membrane.METHODS The clinical data of 263 patients with preterm premature rupture of membranes admitted to Xinxiang Central Hospital from Jun 2020 to Jun 2023 were retrospectively ana-lyzed.According to whether the type of the infection was chorioamniotic infection,the patients were divided into the chorioamniotic infection group and non-chorioamniotic infection group(98 cases/165 cases).The expression levels of TLR4,NF-κB and TRAF6 in placenta were detected by western blot assay and compared between the chorioamniotic infection group and the non-chorioamniotic infection group.The expression levels of TLR4,NF-κB and TRAF6 in placenta of the early,the middle and the late chorioamniotic infection groups were compared.The outcomes of maternal and infant between the chorioamniotic infection group and the non-chorioamniotic infection group were compared.Spearman method was used to analyze the correlation between levels of TLR4,NF-κB,TRAF6 and chorioamniotic infection in patients with preterm premature rupture of membranes.RESULTS The protein levels of TLR4,NF-κB and TRAF6 in placenta of the chorioamniotic infection group were higher than those of the non-chorioamniotic infection group(P<0.05).The levels of TLR4,NF-κB and TRAF6 proteins in the middle-stage and late-stage groups were higher than those in the early-stage group(P<0.05),and the levels of TLR4,NF-κB and TRAF6 proteins in the late-stage group were higher than those in the middle-stage group(P<0.05).The rates of intrauterine distress,postpartum hemorrhage,oligohydramnios,respiratory distress syndrome,neonatal asphyxia and death in the chorioamniotic infection group were higher than those in the non-chorioamniotic infection group(P<0.05).Spearman correlation analysis showed that levels of TLR4,NF-κB,TRAF6 were positively correlated with the stages of chorioamniotic infection(P<0.05).CONCLUSION The ex-pression levels of TLR4,NF-κB and TRAF6 in placenta of patients with preterm premature rupture of fetal mem-brane were high after chorioamniotic infection,which were closely related to the stage of chorioamniotic infection.Chorioamniotic infection in patients with premature rupture of membranes was adverse to maternal and infant out-comes.
