Serum levels of IL-17,IL-12P70 and CD64 in patients with severe pneumonia and their relationship with prognosis
OBJECTIVE To analyze the correlation of serum interleukin(IL)-17,IL-12P70,cluster differentiation antigen 64(CD64)levels with disease severity and prognosis of patients with severe pneumonia.METHODS A total of 120 patients with severe pneumonia and 60 patients with common pneumonia in the Affiliated Hospital of Sha-oxing University of Arts and Sciences between Jan 2021 and Jun 2023 were enrolled as the severe group and com-mon group,respectively.The distribution of pathogens in patients with severe pneumonia were analyzed.The ab-dominal venous blood of 5 ml was collected within 24 hours after admission,and the serum IL-17,IL-12P70 and CD64 levels were detected,and their correlation with score of acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)was analyzed.According to disease outcomes on 28th day,serum levels of IL-17,IL-12P70 and CD64 among patients with different prognosis were compared.Receiver operating characteristic(ROC)curve was used to evaluate the prognostic value of IL-17,IL-12P70 and CD64 in prognosis.RESULTS A total of 138 strains of pathogenic bacteria were isolated from 120 patients with severe pneumonia,mainly gram-negative bacteria.The levels of serum IL-17,IL-12P70 and CD64 levels in the severe group were higher than those in the common group(P<0.05).The levels of serum IL-17,IL-12P70 and CD64 were positively correlated with APACHE Ⅱ score(r=0.437,0.367,0.482;P<0.05).Among 120 patients with severe pneumonia,36 died and 84 survived within 28 days.The serum levels of IL-17,IL-12P70 and CD64 in the survival group were lower than those in the death group(P<0.05).ROC curves analysis showed that area under curve(AUC)of individual and combined detection of serum IL-17,IL-12P70 and CD64 levels for evaluating prognosis of patients were 0.722,0.681,0.680 and 0.843,respectively.CONCLUSION The serum levels of IL-17,IL-12P70 and CD64 significantly increase in pa-tients with severe pneumonia,and their combined detection has high evaluation value for prognosis of patients.
