Pathogenic bacteria and levels of serum immunoglobulins,VCAM-1 and MCP-1 in patients with chronic bacterial prostatitis
OBJECTIVE To explore the characteristics of pathogenic bacteria in patients with chronic bacterial pros-tatitis and their relationship with changes of serum immunoglobulins,vascular cell adhesion molecule-1(VCAM-1)and monocyte chemoattractant protein-1(MCP-1)levels.METHODS A total of 118 patients with chronic bacte-rial prostatitis(as the infection group)and 30 healthy volunteers under physical examination(as the control group)in Lishui Central Hospital from Jun 2021 to Jun 2023 were recruited.The characteristics and drug resist-ance of the main pathogenic bacteria in the infection group were analyzed.Siemens BNII specific protein analyzer was used to detect the levels of serum immunoglobulin(Ig)A,IgG and IgM,and enzyme linked immunosorbent assay was performed to detect serum level of VCAM-1.Serum MCP-1 level was detected by radioimmunoassay.According to the National Institutes of Health chronic prostatitis symptom index,the patients in the infection group were divided into the mild group,moderate group and severe group.The correlation between serum immu-noglobulins,VCAM-1 and MCP-1 levels and disease severity was analyzed.Patients in the infection group were given targeted intervention,and they were classified into the effective group and ineffective group according to the therapeutic efficacy.The value of serum immunoglobulins,VCAM-1 and MCP-1 levels on the evaluation of thera-peutic efficacy of chronic bacterial prostatitis was analyzed.RESULTS Staphylococcus haemolyticus,Staphylococ-cus epidermidis and Staphylococcus aureus were the main gram-positive pathogenic bacteria,and Escherichia coli was the main gram-negative pathogenic bacteria.Serum levels of IgA,IgG,IgM,VCAM-1 and MCP-1 in the in-fection group were higher than those in the control group(P<0.05).The serum levels of IgA,IgG,IgM,VCAM-1 and MCP-1 in patients with chronic bacterial prostatitis were increased with the aggravation of disease condition(P<0.05).The levels of serum IgA,IgG,IgM,VCAM-1 and MCP-1 were higher in the severe group than those in the moderate group and mild group,and these indexes in the moderate group were higher than those in the mild group(P<0.05).The levels of serum IgA,IgG,IgM,VCAM-1 and MCP-1 in patients with chronic bacterial prostatitis were positively correlated with disease severity(P<0.05).After treatment,serum levels of IgA,IgG,IgM,VCAM-1 and MCP-1 in the effective group were lower than those in the ineffective group(P<0.05).The area under curve(AUC)of combined detection of serum immunoglobulins,VCAM-1 and MCP-1 lev-els in evaluating the therapeutic efficacy of patients with chronic bacterial prostatitis was larger than that of the in-dividual detection(P<0.05).CONCLUSION The main pathogenic bacteria of chronic bacterial prostatitis are S.haemolyticus and S.epidermidis,which are highly drug resistant to penicillin and oxacillin.The levels of serum immunoglobulins,VCAM-1 and MCP-1 are related to the disease severity,which have evaluated value on the therapeutic efficacy of the disease.
