摘要
目的 优化感染防控联络护士主导的重症监护室(ICU)耐碳青霉烯类肺炎克雷伯菌(CRKP)感染防控方案并验证其效果.方法 成立研究团队,以多准则决策分析法为理论基础,通过筛选重症监护室耐碳青霉烯类肺炎克雷伯菌感染影响因素、调查其感染防控薄弱点,采用文献调研、专家咨询等方法,优化感染防控联络护士主导的重症监护室耐碳青霉烯类肺炎克雷伯菌防控方案;比较优化方案实施前后各项指标的差异,验证优化方案的临床效果.结果 实施感染防控联络护士主导的重症监护室耐碳青霉烯类肺炎克雷伯菌感染防控优化方案后,耐碳青霉烯类肺炎克雷伯菌感染率降低(P＜0.001);环境检出率下降、ATP检测合格率提升(P＜0.05),手卫生执行情况、多重耐药菌知识调查情况均有所改善.结论 以感染防控联络护士为主导的重症监护室耐碳青霉烯类肺炎克雷伯菌防控优化方案可提升医务人员的防控意识,降低耐碳青霉烯类肺炎克雷伯菌感染的发生率.
Abstract
OBJECTIVE To optimize the infection control link nurses-led intensive care unit(ICU)carbapenem-re-sistant Klebsiella pneumoniae(CRKP)infection prevention and control program and validate its effectiveness.METHODS A research team was set up to optimize the ICLN-led ICU CRKP prevention and control program by screening ICU CRKP infection influencing factors,investigating ICU CRKP infection prevention and control weak-nesses,and adopting methods such as literature research and expert consultation,based on the theoretical basis of multiple criteria decision analysis.To compare the differences in the indicators before and after the implementation of the optimization program,and to verify the clinical effectiveness of the ICLN-led optimization program for in-fection prevention and control of ICU CRKP.RESULTS After the implementation of the ICLN-led ICU CRKP in-fection prevention and control optimization program,the CRKP infection rate of ICU patients was reduced(P＜0.001).The environmental detection rate was reduced,while the qualified rate of ATP test was increased(P＜0.05),and the implementation of hand hygiene and the survey of knowledge of multidrug-resistant bacteria were improved.CONCLUSION Optimizing the ICLN-led CRKP prevention and control program can improve the pre-vention and control awareness of medical staff and reduce the incidence of ICU CRKP infections.
