临床护士预防血液透析导管相关性血流感染的知信行现状及其影响因素

摘要
目的 探讨临床护士在预防血液透析导管相关性血流感染方面的知信行现状,并分析其影响因素,从而制定具有针对性的、个性化的培训方案,减少患者血液透析导管相关性血流感染的发生率.方法 2022年9月,采用便利抽样法选取深圳市33家医院414名从事血液净化工作的临床护士,采用自主设计的知信行调查问卷进行预防血液透析导管相关性血流感染的知信行现状调查,并采用多重线性逐步回归分析法分析其影响因素,探讨血液透析导管相关性血流感染与护士知信行现状之间的关系.结果 414名研究对象在《预防血液透析导管相关性血流感染的知信行问卷》中的总得分为(128.50±14.44)分;其中:知识维度平均(7.42 士 1.20)分、态度维度平均(46.25±8.54)分、行为维度平均(74.82±8.33)分;多重线性逐步回归分析结果表明:职称、工作年限和是否为血液透析专科护士是知识得分的主要影响因素(P＜0.05),学历是影响态度维度得分的主要因素(P＜0.05),行为维度各项均无统计学差异.结论 从事血液净化工作的临床护士在预防血液透析导管相关性血流感染的知信行方面总体水平良好,其中在预防血液透析导管相关性血流感染的态度和行为方面得分较高,但知识得分较低.管理者应更加重视和加强预防血液透析导管相关性血流感染的理论知识方面的学习和培训,提高护士预防感染的相关认知程度,降低患者血液透析导管相关性血流感染的发生率.
Abstract
OBJECTIVE To investigate the current situation of knowledge,belief and practice of clinical nurses in preventing hemodialysis catheter-related bloodstream infection,and to analyze its influencing factors,so as to de-velop targeted and personalized training programs to reduce the incidence of hemodialysis catheter-related blood-stream infection in patients.METHODS In Sep 2022,414 clinical nurses engaged in blood purification work in 33 hospitals in Shenzhen were selected by convenience sampling method.A self-designed questionnaire of knowledge,attitude and practice was used to investigate the current situation of knowledge,attitude and practice in preventing hemodialysis catheter-related bloodstream infection.Multiple linear stepwise regression analysis was used to ana-lyze the influencing factors,and the relationship between hemodialysis catheter-related bloodstream infection and nurses'knowledge,attitude and practice was explored.RESULTS The total score of 414 clinical nurses engaged in blood purification was(128.50±14.44)points in the"knowledge,attitude and practice questionnaire on the pre-vention of hemodialysis catheter-related bloodstream infection".Among them,the average score of knowledge di-mension,attitude dimension and practice dimension were(7.42±1.20)points,(46.25±8.54)points and(74.82±8.33)points.Multiple linear stepwise regression analysis showed that professional title,working years and wheth-er they were hemodialysis specialist nurses were the main influencing factors of knowledge dimension score(P＜0.05).Educational background was the main factor affecting the attitude dimension(P＜0.05),while there was no statistical difference in the behavioral dimension.CONCLUSION Clinical nurses engaged in blood purification work have a good level of knowledge,attitude and practice in preventing hemodialysis catheter-related bloodstream infection.Among them,the scores of attitude and behavior of preventing hemodialysis catheter-associated blood-stream infections are high,but the knowledge score is low.Managers should pay more attention and strengthen the study and training on the theoretical knowledge of preventing hemodialysis catheter-related bloodstream infec-tion,so as to improve nurses'awareness of preventing infection and reduce the incidence of hemodialysis catheter-related bloodstream infection in patients.
