烟曲霉特异性IgE抗体对肺曲霉病的诊断价值

摘要
目的 探讨烟曲霉特异性免疫球蛋白E(Af-sIgE)抗体检测对肺曲霉病(PA)的诊断价值.方法 选取2019年9月-2023年3月就诊于东南大学附属中大医院的PA患者22例为研究组,同期就诊的其他肺部疾病患者22例为对照组,收集两组患者临床资料和实验室检查指标[血清(浆)中Af-sIgE、霉菌混合特异性IgE(mx-sIgE)、总IgE(t-IgE)水平及血常规和C-反应蛋白(CRP)水平],绘制受试者工作特征(ROC)曲线评估诊断价值.结果 研究组患者血清 Af-sIgE、mx-sIgE 和 t-IgE 分别为 7.54(0.53,14.30)kUA/L、2.66(0.47,5.56)kUA/L 和1035.00(360.25,2592.50)IU/ml,抗体浓度均高于对照组(P＜0.05);两组患者血常规及CRP水平比较无统计学差异;ROC曲线分析结果显示,Af-sIgE、mx-sIgE和t-IgE诊断PA的曲线下面积(AUC)值分别为0.945、0.930和0.882,Af-sIgE和mx-sIgE抗体检测诊断PA的敏感度和特异度分别为95.45％、86.36％和86.36％、86.36％.结论 Af-sIgE、mx-sIgE和t-IgE在PA患者中水平升高,均具有较好的诊断效能,有助于PA早期诊断.
Abstract
OBJECTIVE To explore the efficiency of Aspergillus fumigatus specific immunoglobulin E(Af-sIgE)antibody in diagnosis of pulmonary aspergillosis(PA).METHODS A total of 22 patients with PA who were treated in Zhongda Hospital,Southeast University from Sep 2019 to Mar 2023 were retrospectively collected as the study group,meanwhile,22 patients who had other pulmonary diseases were chosen as the control group.The clinical data were col-lected from the two groups of patients,the levels of laboratory test indexes[serum(plasma)Af-sIgE,mixed specific IgE(mx-sIgE),total IgE(t-IgE),blood routine and C-reactive protein(CRP)]were observed and compared between the two groups of patients,and receiver operating characteristic(ROC)curves were drawn to assess the diagnostic efficiency.RESULTS The concentrations of serum Af-sIgE,mx-sIgE and t-IgE antibodies of the study group were respective-ly 7.54(0.53,14.30)kUA/L,2.66(0.47,5.56)kUA/L and 1035.00(360.25,2592.50)IU/ml,higher than those of the control group(P＜0.05);there were no significant differences in the levels of blood routine indexes and CRP between the two groups of patients.The result of ROC curve analysis showed that the areas under curves(AUCs)of Af-sIgE,mx-sIgE and t-IgE were respectively 0.945,0.930 and 0.882 in diagnosis of PA;the sensitivities of Af-sIgE and mx-sIgE antibodies were respectively 95.45％and 86.36％in diagnosis of PA;the specificities were 86.36％and 86.36％,respectively.CONCLUSION The patients with PA show the rise of levels of Af-sIgE,mx-sIgE and t-IgE,the indexes have high diagnostic efficiencies and may facilitate the early diagnosis of PA.
