Infection sites of malignant tumor patients with multidrug-resistant organisms infections and susceptible factors
OBJECTIVE To explore the distribution of infection sites of the malignant tumor patients with multi-drug-resistant organisms(MDROs)infections and analyze the susceptible factors.METHODS Totally 980 patients with malignant tumors who were treated in Qingdao Cancer Hospital from Mar 2019 to Dec 2022 were enrolled in the study,the clinical data of the patients were retrospectively analyzed.The research subjects were divided into the infection group with 196 cases and the non-infection group with 784 cases according to the status of MDROs infections.The incidence rates of MDROs infection,infection sites and spectrum of pathogens were statistically analyzed.Univariate analysis and logistic regression analysis were performed for the susceptible factors for the MDROs infections.RESULTS Among the 980 patients with malignant tumors,196 had MDROs infection,with the incidence 20.00%.Among the patients with MDROs infection,101(51.53%)had respiratory tract infection,and 62(31.63%)had urinary system infection.Totally 201 strains of MDROs were isolated from the submitted specimens of the 196 patients with the infections,among which gram-negative bacteria were dominant,accounting for 72.64%;the proportions of CRAB,CRPA and CRE were 33.83%,22.39%and 16.42%,respectively;the proportion of gram-positive MRSA strains was 27.36%;no strain of VRE was detected in the study.The result of multivariate logistic analysis showed that long bedridden time,no less than 60 years of age,anemia,invasive pro-cedures,complication with no less than 3 types of underlying diseases,use of antibiotics no less than 14 days and body mass index(BMI)no less than 25 kg/m2 were the susceptible factors for MDROs infection in the malignant tumor patients(OR=3.300,3.330,3.501,3.425,2.689,2.291,3.691,P<0.05).CONCLUSION The inci-dence of MDROs infection is high among the malignant tumor patients.The respiratory tract and urinary system are the major infection sites.The gram-negative bacteria are dominant among the pathogens causing the infections.The susceptible factors for the MDROs infection in the malignant tumor patients include anemia,no less than 60 years of age,invasive procedures,complication with no less than 3 types of underlying diseases,use of antibiotics no less than 14 days and body mass index(BMI)no less than 25 kg/m2 and long bedridden time.
