Behaviors for prevention and control of ventilator-associated pneumonia based on intrinsic motivation of medical staff
OBJECTIVE To construct the behavioral mechanism model for prevention and control of ventilator-asso-ciated pneumonia(VAP)for health care workers of intensive care unit(ICU)based on relevant theory of intrinsic motivation so as to provide theoretical bases for improving the health care workers'behaviors of prevention and control of VAP.METHODS A total of 744 health care workers were recruited from ICUs of 22 hospitals in China by convenience sampling method. The impact of intrinsic motivation and extrinsic motivation on ICU health care workers'behaviors for prevention and control of VAP was observed by structural equation model(SEM).RESULTS The SEM was well fitted, with KMO value 0.939,TLI 0.955,CFI 0.896,RMSEA 0.052,and the loading coefficients of all the items were more than 0.6;the intrinsic motivation was positively correlated with the behaviors for prevention and control of VAP(β=0.502,P<0.001),the extrinsic motivation was positively corre-lated with the intrinsic motivation(β=0.195,P<0.001);the extrinsic motivation was not associated with the be-haviors for prevention and control of VAP(β=-0.046,P=0.278).CONCLUSION The intrinsic motivation and extrinsic are the two fundamental powers behind the behaviors for prevention and control of VAP.The intrinsic motivation has a direct positive effect on execution behaviors, and the extrinsic motivation can directly affect the execution behaviors through the positive effect on the intrinsic motivation.
