Etiological characteristics of postoperative pulmonary infection,TNF gene microsatellite polymorphism and expressions of TGF-βi/Smads signaling pathways in esophageal cancer patients
OBJECTIVE To explore the etiological characteristics of postoperative pulmonary infection in the pa-tients with esophageal cancer(EC),plasma tumor necrosis factor(TNF)gene microsatellite polymorphism and expressions of transforming growth factor-β1(TGF-β1)/Smads signaling pathways proteins.METHODS Totally 60 EC patients with postoperative pulmonary infection and 70 EC patients without postoperative pulmonary infec-tion who were treated in Hengshuo People's Hospital from Aug 2020 to Jul 2023 were enrolled in the study and were respectively assigned as the infection group and the no infection group.The clinical data were collected from the EC patients,the etiological characteristics were analyzed,the plasma TNF microsatellite polymorphisms were observed,the expression levels of peripheral blood TGF-β1,Smad2,Smad3 and Smad7 proteins were compared between the two groups of patients.The values of single and joint detection of TGF-β1,Smad2,Smad3 and Smad7 in diagnosis of postoperative pulmonary infection in the EC patients were analyzed.RESULTS Totally 61 strains of pathogens were isolated from the 60 EC patients of the infection group,67.21%of which were gram-negative bac-teria,26.23%were gram-positive bacteria,6.56%were fungi;Klebsiella pneumoniae and Pseudomonas aerugi-nosa were dominant among the gram-negative bacteria;Staphylococcus aureus was the predominant species of the gram-positive bacteria.The proportions of the patients with TNF gene microsatellite TNFc 1/1 genotype and TN-Fc1 allele were higher in the infection group than in the no infection group(P<0.05).The levels of peripheral blood TGF-β1,Smad2,Smad3 and Smad7 proteins of the infection group were higher than those of the no infec-tion group(P<0.05).The area under curve(AUC)of the joint detection of TGF-β1,Smad2,Smad3 and Smad7 was higher than that of the single detection in the diagnosis of postoperative pulmonary infection in the EC pa-tients,the specificity of the joint detection was 87.14%,with the sensitivity 86.67%.CONCLUSION K.pneu-moniae,P.aeruginosa and S.aureus are dominant among the pathogens isolated from the EC patients with post-operative pulmonary infection.The postoperative pulmonary infection of the EC patients is associated with the per-centages of plasma TNF gene microsatellite TNFc1/1 genotype,TNFc1 allele and the levels of TGF-β1/Smads signaling pathway proteins.The joint detection of TGF-β1,Smad2,Smad3 and Smad7 had high value in the diag-nosis of postoperative pulmonary infection in the EC patients.
