摘要

目的 评价医院感染预防与控制(简称感控)专职人员的核心能力现况,分析其影响因素并探讨其与自我效能感之间的关联程度.方法 选取江苏省镇江市、常州市和盐城市各医疗机构感控专职人员431名为研究对象,采用一般资料调查表、感控专职人员核心能力问卷和一般自我效能感量表进行问卷调查.结果 单因素分析结果显示,年龄、医院性质、医院等级、医院规模(床位数)、职称、工作年限、从事感控工作年限、个人月收入、有无外出进修、每年参加感控学习次数、医院对感控工作支持力度、医院感控氛围对感控专职人员的核心能力有显著影响(P<0.05);多因素分析结果显示,年龄、医院规模(床位数)、医院感控氛围、从事感控工作年限、每年外出感控学习次数、医院对感控工作支持力度是感控专职人员核心能力的影响因素,共可解释感控专职人员核心能力总变异的86.5%;Spearman相关性分析结果显示,自我效能感与核心能力总分以及各维度评分均呈中等正相关(rs=0.4521~0.5770,P<0.0001).结论 江苏省感控专职人员的核心能力处于良好水平,但其队伍的专业结构有待优化,在医院感染风险预测能力、感控专业发展能力方面仍有提升空间,医疗机构应加强对医院感控工作的支持力度,强化其体系和内涵建设,以不断提高感控专职人员的核心能力.

Abstract

OBJECTIVE To evaluate the current status of core competence of the infection control professional,ana-lyze the influencing factors and observe the association with self-efficacy.METHODS A total of 431 infection con-trol professionals were recruited from the medial institutions in Zhenjiang,Changzhou and Yancheng of Jiangsu Province.The questionnaire survey was conducted by using baseline questionnaire,questionnaire for competence of infection control professionals and general self-efficacy scale.RESULTS The result of univariate analysis showed that the age,nature of hospital,hospital grade,scale of hospital(number of beds),title,years of working,per-sonal income,engagement of further study,number of times of participating infection control meetings in each year,support for infection control from the hospital and atmosphere of infection control had remarkable effect on the core competence of the infection control professionals(P<0.05).The result of multivariate analysis indicated that the age,scale of hospital(number of beds),atmosphere of infection control,years of engaging for infection control,times of participating infection control meetings in each year and the hospital's support for infection con-trol were the influencing factors for the core competence of the infection control professionals,and 86.5%of the total variation of core competence of the infection control professionals could be explained in total.The result of Spearman correlation analysis showed there was medium positive correlation between the self-efficacy and the total score of core competence and dimensions(rs=0.4521-0.5770,P<0.0001).CONCLUSION The core competence of the infection control professionals of Jiangsu Province is at a high level,however,the professional structure of the teams needs to be optimized,the capability of risk prediction for nosocomial infection and the ability of devel-opment of infection control need to be further improved.It is necessary for the medical institutions to reinforce the support for the infection control,and intensify the construction of system and connotation so as to continuously boost the core competence of the infection control professionals.
