Application of risk assessment in the management of nosocomial infections in clinical departments
OBJECTIVE To explore the application effect of risk assessment in the management of nosocomial infec-tions in clinical departments.METHODS The literature review and brainstorming method was adopted to identify the risk factors of nosocomial infection in clinical departments in Tongji Hospital.Then the risk assessment indica-tors system of nosocomial infection was determined.Based on the baseline data of nosocomial infection surveil-lance,the risk occurrence possibility of each indicator was assigned a unified score.The expert consultation meth-od was adopted to assign values to the severity of consequences and the integrity of the current system.The overall risk assessment scores of each indicator and each clinical department were calculated,and the percentile method was used to evaluate the risk grade.RESULTS Of the 41 clinical departments evaluated,the four departments with an overall score of very high risk were intensive care unit,cardiac vascular surgery,neurosurgery,and thoracic surgery.The distributions of departments with extremely high risk ratings in management indicators,process in-dicators and outcome indicators varied,with intensive care unit being a very high risk department in all three cate-gories.The differences in risk assessment scores among the 6 risk groups for process and outcome indicators were significantly different(P<0.05).Of the 34 identified risk indicators,3 extremely high risk indicators were failure to perform hand hygiene,occurrence of nosocomial infection,and failure to perform pre-treatment pathogenetic delivery of restricted-use-grade antimicrobials.CONCLUSIONS The whole-hospital risk assessment of clinical de-partments can screen out high-risk departments and links.We can use limited resources to focus on key depart-ments,scientifically prevent important risks and achieve precise nosocomial infection prevention and control.
