Association of Bcl-2,Bax,NLR and TLR3 gene polymorphisms with concurrent lung infections in elderly patients with cerebral infarction
OBJECTIVE To investigate the association of B lymphocytoma gene-2 protein(Bcl-2),B lymphocytoma gene-2-related X protein(Bax),neutrophil and lymphocyte ratio(NLR)in peripheral blood and Toll-like receptor 3(TLR3)gene polymorphisms with concurrent lung infections in elderly patients with cerebral infarction.METHODS The clinical data of 159 elderly patients with cerebral infarction admitted to the Affiliated Hospital of Weifang Medical College from Mar.2022 to Oct.2023 was retrospectively analyzed,and the patients were as-signed into the infected group with 46 cases and the uninfected group with 113 cases according to the status of lung infection.The clinical characteristics,the distributions of Bcl-2,Bax,NLR and TLR3 gene polymorphisms in pe-ripheral blood were compared between the two groups,the predictive values of single and combined detection of Bcl-2,Bax and NLR to the onset of pulmonary infection in elderly patients with cerebral infarction was analyzed.RESULTS Proportions of smoking,swallowing dysfunction,invasive maneuvers,combined diabetes mellitus in the infected group were higher than those in the uninfected group(P<0.05),serum Bax and peripheral blood NLR levels in the infected group were higher than those in the uninfected group(5.22±1.47)μg/L vs.(4.13±0.95)μg/L,(6.13±1.72)μg/L vs.(4.26±1.08)µg/L,respectively)(P<0.05).Serum Bcl-2 level in the in-fected group was lower than that of uninfected group(0.98±0.31)μg/L vs.(1.25±0.40)μg/L)(P<0.05).The frequency of AA genotype and A allele at rs5743303 locus of TLR3 gene in the infected group were 63.04%and 70.65%,respectively,which were higher than those in the uninfected group,which were 37.17%and 52.21%(P<0.05).The area under the receiver operating characteristic curve(AUC)of the joint model of serum Bcl-2,Bax,and peripheral blood NLR for prediction of concomitant lung infections in elderly patients with cere-bral infarction was 0.915,the sensitivity and specificity were 91.30%and 84.96%,respectively.CONCLUSIONS The occurrence of pulmonary infection in elderly patients with cerebral infarction was related to TLR3 polymor-phism and Bax,NLR and BCL-2 expression.Combined usage of serum BCL-2,Bax and NLR in peripheral blood is more valuable than the single indicator in predicting pulmonary infection in elderly patients with cerebral infarc-tion.
