Expression of serum JAK2/STAT3 signaling pathway,TLR and drug resistance of pathogenic bacteria in patients with severe pneumonia
OBJECTIVE To investigate the serum expression of Janus kinase 2(JAK2)/signal transduction and transcriptional activator 3(STAT3)signaling pathway and Toll-like receptor(TLR)and to analyze the character-istics and drug resistance of pathogenic bacteria in patients with severe pneumonia.METHODS A total of 97 pa-tients with severe pneumonia admitted to our hospital from Nov.2020 to Nov.2023 were selected as the study group,and other 98 patients with common pneumonia in the same period were selected as the control group.The distribution and drug resistance of pathogenic bacteria were analyzed.The expressions of JAK2,STAT3,TLR2 and TLR4 in serum were compared between the two group and their predictive values for severe pneumonia were analyzed by receiver operating characteristic curve(ROC).RESULTS A total of 108 strains of pathogenic bacteria were isolated from the study group,of which 74 strains of gram-negative accounted for 68.52%.In the main gram-negative bacteria,Acinetobacter baumannii strongly resisted to piperacillin and ceftazidime with resistant rate>70%,but was sensitive to minocycline,colistin and tigecycline with resistant rate<10%.Pseudomonas aeruginosa had strong resistance to piperacillin with resistant rate>34.62%,weak resistance to amikacin,mino-cycline and tobramycin with resistant rate<10%,and was sensitive to colistin.The main gram-positive bacteria Staphylococcus aureus had strong resistance to penicillin with resistant rate 90.48%,and was sensitive to linezol-id,vancomycin and kauranen.The serum levels of JAK2,STAT3,TLR2 and TLR4 in the study group were all significantly higher than those in the control group(P<0.05).The area under the receiver operating characteristic curve(AUC)of combined JAK2,STAT3,TLR2 and TLR4 in the diagnosis of severe pneumonia was higher than that of each single indicators with the sensitivity 78.40%and specificity 92.90%(P<0.05).CONCLUSIONS Most patients with severe pneumonia are infected by gram-negative bacteria.A.baumannii and S.aureus have high re-sistant rate to common antibiotics.During severe pneumonia,JAK2/STAT 3 signal pathway was activated and TLR2 and TLR4 increased;combined detection of those indicators is helpful in the diagnosis of severe pneumonia.
