目的 分析2型糖尿病合并幽门螺杆菌(Hp)感染影响因素及其心血管并发症。方法 选取2021年11月-2023年2月于衡水市人民医院心血管内科接受治疗的2型糖尿病患者168例作为研究对象;所有患者入院后均实施13C尿素呼气试验,并根据试验结果分为Hp感染组(n=78)和无Hp感染组(n=90);比较两组患者心血管并发症情况,分析患者临床资料,采用多因素Logistic回归分析归纳2型糖尿病患者发生Hp感染的影响因素。结果 Hp感染组患者糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)及丙二醛(MDA)水平分别为(8。66±1。58)%、(5。11±1。06)mmol/L、(2。28±0。44)mmol/L、(3。36±0。49)mmol/L、(7。22±2。20)mmol/ml 高于无 Hp 感染组(P<0。05),高密度脂蛋白胆固醇(HDL-C)及超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)为(1。01±0。22)mmol/L、(33。65±5。69)U/ml、(58。69±8。96)U低于无Hp感染组(P<0。05);Hp感染组患者心血管并发症发生率(10。26%)高于非Hp感染组(P=0。028);HbA1c(OR=2。787,95%CI:1。694~4。585)、SOD(OR=2。664,95%CI:1。800~3。943)、MDA(OR=3。931,95%CI:1。874~8。247)、GSH-PX(OR=3。059,95%CI:1。739~5。379)是 2 型糖尿病患者 Hp 感染的影响因素(P<0。05)。结论 2型糖尿病患者合并Hp感染可增加血管不良事件发生率,另外检测血清HbA1c、SOD、MDA、GSH-PX有助于评估2型糖尿病患者Hp感染发生。
Influencing factors of type 2 diabetes mellitus combined with Helicobacter pylori infection and its cardiovascular complications
OBJECTIVE To analyze the influencing factors of type 2 diabetes mellitus combined with Helicobacter pylori(Hp)infection and its cardiovascular complications.METHODS A total of 168 patients with type 2 diabetes di-agnosed and treated in the Department of Cardiovascular Medicine of Hengshui People's Hospital from Nov.2021 to Feb.2023 were selected as the study subjects.All patients underwent 13C urea breath test after admission,and were divided in-to Hp-infected group(n=78)and non-Hp-infected group(n=90)according to the test results.The cardiovascular com-plications were analyzed and compared between the two groups,the clinical data of patients were analyzed,and the influ-encing factors of Hp infection in type 2 diabetes patients were summarized by multivariate logistic regression analysis.RESULTS The levels of glycated hemoglobin(HbA1c),total cholesterol(TC),triglycerides(TG),low-density lipopro-tein cholesterol(LDL-C),and malondialdehyde(MDA)in patients of the Hp-infected group were(8.66±1.58)%,(5.11±1.06)mmol/L,(2.28±0.44)mmol/L,(3.36±0.49)mmol/L,and(7.22±2.20)mmol/ml,respective-ly,which were higher than those of the non-Hp-infected group(P<0.05)while the levels of high-density lipo-protein cholesterol(HDL-C),superoxide dismutase(SOD),and glutathione peroxidase(GSH-PX)were(1.01±0.22)mmol/L and(33.65±5.69)U/ml and(58.69±8.96)U,respectively,lower than those of the non-Hp-infected group(P<0.05);The incidence of cardiovascular complications in patients of the Hp-infected group(10.26%)was higher than that of the non-Hp-infected group(P=0.028);HbA1c(OR=2.787,95%CI:1.694-4.585),SOD(OR=2.664,95%CI:1.800-3.943),MDA(OR=3.931,95%CI:1.874-8.247),GSH PX(OR=3.059,95%CI:1.739-5.379)were the influencing factors of Hp infection in type 2 diabetes patients(P<0.05).CONCLUSION Comorbid HP infection in patients with type 2 diabetic mellitus increased the incidence of adverse vascular events,and additional detection of serum HbA1c,SOD,MDA,GSH-PX was helpful in asses-sing the occurrence of HP infection in patients with type 2 diabetic mellitus.
