Risk factors for Escherichia coli infection and its drug resistance among pregnant women
OBJECTIVE To explore the risk factors for Escherichia coli infection and its drug resistance among pregnant women.METHODS The clinical data of a total of 6,337 cases of pregnant women who were hospitalized and underwent microbiological testing due to premature rupture of membranes,threatened premature delivery,threatened abortion,fever and other reasons from Jan.2020 to Dec.2022 in Obstetrics and Gynecology Hospital of Fudan University were retrospectively analyzed,the types and distribution of pathogenic microorganisms were an-alyzed,and the risk factors for Escherichia coli infection were summarized.RESULTS A total of 925 strains of pathogenic bacteria were co-cultured and isolated,among which the top three strains were 231 strains of Candida alba(24.97%),202 strains of Escherichia coli(21.84%),and 171 strains of Streptococcus agalactis(18.49%),and the detection rate of microorganisms and Escherichia coli in the last three years was 14.60%and 3.19%.Parturient women(OR=0.349,95%CI:0.222-0.549,P<0.001)was a protective factor for E.coli infection,while BMI(OR=1.419,95%CI:1.061-1.897,P=0.018)was a risk factor for E.coli infection.The resistance rate of Escherichia coli to compound sulfamethoxazole,ceftriaxone,ciprofloxacin,cefazoline,am-picillin/sulbactam was more than 25%,while the resistance rate to furanotoin,cefepime and ceftazidime was less than 10%.No strains resistant to piperacillin/tazobactam,cefotetan,ertabenem,imipenem and amicacin were de-tected.CONCLUSION Escherichia coli was an important pathogen causing infections during pregnancy,clinicians should to grasp the high-risk factors of Escherichia coli infections and select sensitive antibiotics for anti-infective treatment according to the characteristics of drug resistance.
