Pathogenic bacteria,drug resistance and expression of TLR2 and TLR4 in patients with lung cancer complicated by pulmonary infection
OBJECTIVE To explore the pathogenic bacteria and drug resistance as well as the expression of Toll like receptor(TLR)2 and TLR4 in patients with lung cancer complicated by pulmonary infection(PI).METHODS Totally 119 patients with lung cancer complicated by PI admitted to Jiangsu Provincial People's Hospital Chongqing Hospital(Qijiang District People's Hospital)from Oct.2021 to Oct.2023 were selected as the group A,and 122 patients with lung cancer only during the same period were as the group B.The pathogenic bacteria were analyzed,the protein and messenger ribonucleic acid(mRNA)levels were tested by Western blot and fluo-rescence quantitative polymerase chain reaction(FQPCR),and the diagnostic value was analyzed by receiver oper-ating characteristic(ROC)curve.RESULTS In the study group,gram-negative bacteria accounted for a high pro-portion,which mainly were Pseudomonas aeruginosa and Escherichia coli;P.aeruginosa was highly resistant to piperacillin/tazobactam,ampicillin/sulbactam,cefepime,ceftazidime,levofloxacin,and aztreonam,with the re-sistance rates 81.82%,86.36%,68.18%,77.27%,68.18%,and 68.18%,respectively.Levels of proteins and mRNA for TLR2 and TLR4 in the group A were higher than those in group B(P<0.05).Area under the curves(AUC)of TLR2 protein and mRNA,TLR4 protein and mRNA and the combination were all high for diagnosing lung cancer complicated by PI with high sensitivities and specificities,and the combination was the highest among them(P<0.05).CONCLUSIONS In lung cancer with PI,the expressions of TLR2 and TLR4 are upregulated;the combined detection of TLR2 and TLR4 proteins and mRNAs shows a high diagnostic value for the complication.Meanwhile,gram-negative bacteria P.aeruginosa is the main pathogen with high resistance to the antibiotics commonly used in clinic.Therefore,the detection of TLR2 and TLR4,the distribution and drug resistance of the pathogenic bacteria can help clinicians to provide a targeted treatment and intervention in time and improve the prognosis of the patients.
