Factors influencing spontaneous seroclearance of HBsAg in HBeAg negative chronic HBV infection
OBJECTIVE To analyze the factors influencing the spontaneous seroclearance of serum HBsAg in HBeAg-negative chronic hepatitis B virus(HBV)infected population without antiviral therapy,so as to raise the awareness of HBV infection and improve the curative effect.METHODS A total of 320 patients with HBeAg-nega-tive chronic HBV infection were selected from the outpatient department of the Fifth Medical Center of the Chi-nese PLA General Hospital from Feb.2012 to Sep.2023.The general data and serological indicators were collected and statistically analyzed.RESULTS In the 320 cases,the quantitative HBsAg level was negatively correlated with age(r=-0.325,P<0.0001)and positively correlated with HBV DNA level(r=0.333,P<0.0001);mean-while,HBsAg level in female was significantly higher than that in male(z=-4.435,P<0.0001).All of the study cases were followed up for a total 346.25 person-years,while 29 cases performed HBsAg clearance corre-sponding to an annual rate of 8.38%;multivariate Cox regression analysis showed that baseline HBsAg level was an independent factor for the spontaneous seroclearance of HBsAg in HBeAg negative chronic HBV infection(P<0.05).The spontaneous and cumulative seroclearance rates of HBsAg in the elderly group were significantly higher than those in the young groups(P<0.05),and the cumulative rate of HBsAg in male was higher than that in fe-male(P<0.05).The lower the baseline HBV DNA and HBsAg levels were,the higher the spontaneous HBsAg seroclearance rate was.In patients with baseline HBV DNA<40 IU/ml and HBsAg<10 IU/ml,the annual and cumulative seroclearance rate of HBsAg were 19.06%and 90.29%,20.00%and 87.29%,respectively.CONCLUSION The elderly and baseline HBV DNA<40 IU/ml are the influencing factors for spontaneous sero-clearance HBsAg in HBeAg-negative chronic HBV infected population;furthermore,quantitative baseline HBsAg level is an independent factor and HBsAg<10 IU/ml is a predictive factor for spontaneous seroclearance of HB-sAg,which has important clinical significance.
