Literature review of 1 case of intracranial infection with Purpureocillum illiacium
OBJECTIVE Purpureocillium lilacinum(P.lilacinum)is a rare fungal pathogen,mainly seen in im-munosuppressed patients.There is currently a lack of evidence-based treatment guidelines.In this study,a case of an immunodeficient patient with intracranial infection caused by P.lilacinum was analyzed to understand the dis-ease characteristics,treatment and outcome of patients with intracranial infection caused by P.lilacinum.METHODS In Dec.2023,the rheumatology department of Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology detected P.lilacinum in the cerebrospinal fluid of a patient with systemic lupus erythematosus encephalopathy by applying next-generation sequencing technology.This study ana-lyzed the clinical data of the first patient with intracranial infection caused by P.lilacinum on a global scale,ai-ming at the clinical symptoms and infection of the patient.Based on the relevant literature at domestic and overseas as the basis of medication,the adjustment of treatment plan was carried out,and the analysis of treatment plan was targeted.RESULTS A case of intracranial infection with rare fungus P.lilacinum,and accompanied by bacte-rial and viral infection was clinically detected.Fluconazole was not effective at the beginning of antifungal treat-ment.Clinical pharmacists adjusted the administration schedule according to the literature and selected posacon-azole for antifungal infection treatment,and the infection symptoms of the patient were controlled.The infection did not recur during 3 months of follow up.CONCLUSION Posaconazole was effective in this case of intracranial infection caused by P.lilacinum after fluconazole treatment failure.
