高危型HPV感染与宫颈病变组织中cyClinD1、Ki-67和HER-2蛋白表达的关联

目的 探究高危型人乳头瘤病毒(HPV)感染与宫颈病变组织中细胞周期素D1(cyClinD1)、细胞增殖核抗原(Ki-67)和人表皮生长因子受体-2(HER-2)蛋白表达的关联。方法 选取萍乡妇幼保健院2019年1月-2023年12月收治的81例宫颈病变患者为研究对象,根据患者宫颈病变情况分为子宫颈低级别鳞状上皮内病变(LSIL)24例、子宫颈高级别鳞状上皮内病变(HSIL)30例和宫颈癌27例。比较三种宫颈病变组织的高危型HPV感染阳性率,并根据HPV阳性结果分为感染组与非感染组,比较两组患者宫颈病变组织cyClinD1、Ki-67和HER-2蛋白表达情况,并分析高危型HPV感染与cyClinD1、Ki-67和HER-2蛋白表达对宫颈病变的诊断效果。结果 LSIL、HSIL和宫颈癌三种宫颈病变组织的高危型HPV感染阳性率呈LSIL<HSIL<宫颈癌(P<0。05)。感染组宫颈病变组织cyClinD1、Ki-67和HER-2蛋白表达阳性率高于非感染组(P<0。05)。高危型HPV感染与cyClinD1、Ki-67和HER-2蛋白表达评估LSIL的一致率分别为83。33%、79。17%和46。43%(kappa值分别为 0。673、0。600 和 0。146);评估 HSIL 的一致率分别为 86。67%、96。67%和 73。33%(kappa 值分别为 0。595、0。870和 0。375);评估宫颈癌的一致率分别为 96。30%、96。30%和 85。19%(kappa 值分别为 0。780、0。649 和 0。438)。结论 高危型HPV感染阳性患者的宫颈病变组织中cyClinD1、Ki-67和HER-2蛋白表达率高于阴性患者,且对于宫颈组织的病变程度具有一定的诊断效果。
Relationships between high-risk HPV and protein expressions of cyClinD1,Ki-67 and HER-2 in cervical lesions
OBJECTIVE To investigate the relationships between high-risk human papillomavirus(HPV)and the protein expressions of cell cycle protein(cyClinD1),nuclear associated antigen(Ki-67)and human epidermal growth factor receptor 2(HER-2)in cervical lesions.METHODS This retrospective study included 81 patients with cervical lesions diagnosed at Pingxiang Maternal and Child Health Hospital from Jan.2019 to Dec.2023,in-cluding 24 cases of low-grade cervical squamous intraepithelial lesions(LSIL),30 cases of high-grade cervical squamous intraepithelial lesions(HSIL)and 27 cases of cervical cancer.Clinical data and tissue samples were col-lected.HPV genotyping,cyClinD1,Ki-67 and HER-2 proteins were tested and statistically analyzed.RESULTS The positive rates of high-risk HPV in the three types of cervical lesion tissues were as follows:LSIL<HSIL<cervical cancer(P<0.05).The protein expressions of cyClinD1,Ki-67 and HER-2 in HPV infected cervical lesion tissues were significantly higher than those in non-infected ones(P<0.05).The concordance rate of high-risk HPV with cyClinD1,Ki-67 and HER-2 protein levels in LSIL was 83.33%,79.17%and 46.43%with kappa val-ues 0.673,0.600 and 0.146,respectively.The concordance rate of high risk HPV with cyClinD1,Ki-67 and HER-2 protein levels in HSIL was 86.67%,96.67%,73.33%with kappa values 0.595,0.870,0.375,respectively.The concordance rate of high risk HPV with cyClinD1,Ki-67 and HER-2 protein levels in cervical cancer was 96.30%,96.30%and 85.19%with kappa values 0.780,0.649 and 0.438,respectively.CONCLUSION CyClinD1,Ki-67 and HER-2 proteins in high-risk HPV-positive cervical lesion tissues express higher than those in negative tissues,which has certain diagnostic value for assessing the tumorigenic stage of cervical lesions.
