Current status of the diagnosis of statutory infectious diseases and survey of requirement for symptom monitoring and early warning system
OBJECTIVE To survey the misdiagnosis and missed diagnosis of statutory infectious diseases made by clinicians and to investigate their opinions and needs on key points of the symptom monitoring and early warning system.METHODS An electronic questionnaire was designed and generated by the questionnaire star network plat-form,and distributed to 180 clinicians through the WeChat group.The results were collected and an Excel data-base was constructed and analyzed by SPSS 27.0 software.The basic information of the respondents was descrip-tively analyzed.The percentage,penetration rate and response rate of multi-choice questions were calculated and tested by Pareto chart analyses and chi-square goodness of fit tests.In the Pareto chart analysis,the cumulative composition ratio in the range of 0 to 80%was the main factor,and 80%to 100%was the general factor.Chi-square goodness of fit tests analyzed the statistical difference of the selected proportions of each item.RESULTS A total of 128 valid questionnaires were collected with a participation rate of 71.11%.Among the participants,atten-ding physicians accounted for 25.80%and deputy chief and chief physicians for 62.50%.The participants had rich clinical experience and could adequately answer questions about the status of the diagnosis for infectious disease.Totally 40 clinicians were from the department of infectious diseases,among whom 25 had missed or misdiagnosed one of the 41 statutory infectious diseases once in their careers.Pulmonary tuberculosis and infectious diarrhea were most likely to be missed or misdiagnosed;and the main reasons were lack of experience,specific clinical manifestations and laboratory diagnosis.The symptom monitoring and early warning system for statutory infec-tious diseases were required by 82.80%of the participants,which should have real-time warning function like the pop-up window of the computer interface and Hospital information system(HIS)patient list reminder to send messages to the reception,competent,on duty,HIS system online and infection management doctors.CONCLUSIONS The official infectious diseases,especially tuberculosis and infectious diarrhea,are difficult to di-agnose accurately.The accuracy can be improved by accumulating clinical experience,capturing the specific clinical manifestation of the disease in time and improving corresponding laboratory tests.
