摘要
目的 分析充气式加温仪在不同温度设置对腹腔镜下胆囊切除术患者护理的影响.方法 本研究选取了 2021年1月至2023年1月连云港市第二人民医院收治的50例腹腔镜下胆囊切除术患者,利用随机数字表格法分组,术中均使用充气式加温仪保温,对照组25例设置温度为32℃、观察组25例设置温度为38℃,观察两组患者中心温度变化、并发症发生情况、麻醉苏醒镇静评分、胃肠功能恢复情况.结果 观察组入手术室前、麻醉5min后患者中心温度与对照组比较,差异无统计学意义(P＞0.05);麻醉30min、麻醉60min、手术结束时观察组中心温度均高于对照组,差异有统计学意义(P＜0.05).观察组手术时间、术中出血与对照组比较,差异无统计学意义(P＞0.05);肛门排气时间、肠鸣音恢复时间、排便时间观察组均低于对照组,差异有统计学意义(P＜0.05).观察组在各个不同时间点的麻醉苏醒镇静评分(Ramsay),评分均高于对照组,差异有统计学意义(P＜0.05).观察组腹痛、切口感染、恶心呕吐等发生率与对照组比较,差异无统计学意义(P＞0.05);而观察组躁动、寒战发生率低于对照组,差异有统计学意义(P＜0.05).结论 腹腔镜下胆囊切除手术患者术中设置38℃充气式加温仪,能够维持患者术中稳定的中心体温,预防寒战、躁动,有利于术后胃肠功能恢复,降低患者应激程度,值得推广.
Abstract
Objective To analyze the effect of different temperature settings on inflatable heating apparatus on the nursing care of patients with laparoscopic cholecystectomy.Methods 50 patients with laparoscopic cholecystectomy from January 2021 to January 2023 were enrolled in this study,the setting temperature of the 25 cases in the control group was 32 ℃,and that of the 25 cases in the observation group was 38℃.The changes of central temperature,the occurrence of complications,the score of sedation,and recovery of gastrointestinal function were observed.Results The central temperature of the patients in the observation group was higher at 30 min,60 min,and at the end of operation than that in the control group,the differences were statistically significant(P＜0.05),while the central temperature at 5 min and at the beginning of operation of the two groups were not statistical significant(P＞0.05).Compared with the control group,the observation group had no statistically significant difference in operation time,bleeding during operation(P＞0.05),and the observation group had lower anal exhaust time,bowel sound recovery time,and defecation time than those of the control group,the difference was statistically significant(P＜0.05).Ramsay score of the observation group was higher than that of the control group at every different time points(P＜0.05).The incidences of abdominal pain,incision infection,nausea,and vomiting in the observation group were not significantly different from those in the control group(P＞0.05),but the incidences of restlessness and shivering in the observation group were significantly lower than those in the control group(P＜0.05).Conclusion The application of 38 ℃ inflatable warming instrument during laparoscopic cholecystectomy can maintain the stable central body temperature,prevent chills and agitation,and is beneficial to the recovery of gastrointestinal function,reduce the degree of patients with stress,it is worth promoting.
