摘要
目的 分析急诊急救输血中红细胞不规则抗体筛查的应用价值.方法 回顾性选取厦门市湖里区妇幼保健院2019年1月至2023年12月期间收治的4898例拟输血患者资料进行分析,收集患者实施红细胞不规则抗体筛查结果和临床输血治疗情况.结果 对不规则抗体筛查结果进行统计,4898例拟输血患者中,共有72例患者不规则抗体筛查结果呈阳性,所占比例为1.47％(72/4898).72例不规则抗体筛查结果阳性患者中,65(90.28％)例患者有妊娠史,6例(8.33％)有输血史,28例(38.89％)患者合并肿瘤疾病,26例(36.11％)患者合并血液疾病,11例(15.28％)患者合并其他疾病.72例不规则抗体筛查结果阳性患者中,40例获得匹配的血液制品,顺利完成急诊急救输血治疗,未出现任何不良反应;20例因交叉反应导致筛查结果呈现出假阳性;12例未找到合适配血者,未接受输血治疗.结论 急诊急救输血治疗中,对拟输血患者实施红细胞不规则抗体筛查可以及时掌握患者情况,为临床血液的选择提供参考,减少输血后不良反应的发生,确保输血安全.
Abstract
Objective To analyze the application value of screening for irregular red blood cell antibodies in emergency blood transfusion.Methods A retrospective analysis was conducted on the data of 4898 prospective blood transfusion patients admitted to Huli District Maternal and Child Health Hospital in Xiamen from January 2019 to December 2023.The results of red blood cell irregular antibody screening and the information of clinical blood transfusion treatment were collected.Results According to the statistics of irregular antibody screening results,out of 1125 patients who planned to undergo blood transfusion,72 patients tested positive for irregular antibody screening,accounting for 1.47％(72/4898).Among the 72 patients with positive irregular antibody screening results,65 patients(90.28％)had a history of pregnancy,6 patients(8.33％)had a history of blood transfusion,28 patients(38.89％)had tumor diseases,26 patients(36.11％)had blood diseases,and 11 patients(15.28％)had other diseases.Among 72 patients with positive irregular antibody screening results,40 received matching blood products and successfully completed emergency blood transfusion treatment without any adverse reactions;20 cases showed false positive screening results due to cross reactivity;12 cases did not find suitable blood donors and did not receive blood transfusion treatment.Conclusion In emergency blood transfusion treatment,screening for irregular red blood cell antibodies in patients who plan to receive blood transfusion can timely grasp the patient's condition,provide reference for clinical blood selection,reduce the occurrence of adverse reactions after transfusion,and ensure blood transfusion safety.
