逆行腓肠神经营养血管皮瓣修复足踝部软组织缺损临床观察

摘要
目的 探讨对足踝部软组织缺损患者采用逆行腓肠神经营养血管皮瓣修复方案的临床效果.方法 纳入青岛市黄岛区区立医院骨科的80例足踝部软组织缺损患者作为研究对象,随机分为逆行腓肠神经营养血管皮瓣修复(研究组)、股前外侧皮瓣修复(对照组)两个组,两组各40例.对比分析术后患者炎性因子指标,随访足踝部软组织缺损患者对术后外观满意度、皮瓣成活率及修复部位皮肤弹性.结果 两组术前炎性因子评分无统计学意义(P＞0.05),治疗后研究组低于对照组(P＜0.05)有统计学意义;研究组治疗后非常满意(42.50％)、总满意率(97.50％)均明显高于对照组(12.50％,77.50％)(P＜0.05);研究组治疗后皮瓣成活率39例(97.50％)高于对照组28例(70.00％),修复部位皮肤弹性改善且效果好于对照组(P＜0.05).结论 将腓肠神经营养血管皮瓣修复方式用于修复足踝部软组织缺损患者,能够改善炎性因子指标,提升患者对术后外观的满意度,提高皮瓣成活率,有助于修复部位皮肤弹性,安全性更高.
Abstract
Objective To explore the clinical effect of using a retrograde sural nerve nutrient vascular flap repair scheme for patients with soft tissue defects in the foot and ankle.Methods Eighty patients with soft tissue defects in the foot and ankle treated in the orthopedics department of Huangdao District Hospital in Qingdao were selected as the research subjects.They were randomly divided into the retrograde sural nerve nutrient vessel flap repair group(research group)and the lateral femoral flap repair group(control group),with 40 cases in each group.We comparatively analyze the inflammatory factor indicators in postoperative patients,follow-up of patients with soft tissue defects in the foot and ankle,satisfaction with postoperative appearance,flap survival rate,and skin elasticity at the repair site.Results There was no statistically significant difference in preoperative inflammatory factor scores between the two groups(P＞0.05),and after treatment,the score of the study group was lower than that of the control group(P＜0.05);After treatment,the research group's very satisfied rate(42.50％)and the overall satisfied rate(97.50％)was significantly higher than those of the control group(12.50％,77.50％)(P＜0.05);After treatment,the survival rate of skin flaps in the study group was 97.50％(39 cases),which was higher than that in the control group of 70.00％(28 cases).The elasticity of the repaired skin improved in the research group was better than that in the control group.The statistical results showed a significant difference(P＜0.05).Conclusion The use of the sural nerve nutrient vessel flap repair method for patients with soft tissue defects in the foot and ankle can improve inflammatory factor indicators,enhance patient satisfaction with postoperative appearance,increase the survival rate of the flap,help repair the skin elasticity of the repair site,and have higher safety.
