紧急宫颈环扎术联合盐酸利托君在宫颈机能不全性晚期流产中的临床应用

摘要
目的 探讨对宫颈机能不全性晚期流产患者予以紧急宫颈环扎术联合盐酸利托君治疗的效果.方法 357例宫颈机能不全性晚期流产患者来源于2019年12月至2023年12月福建省漳州市医院,按照是否实施手术划分为研究组198例(紧急宫颈环扎术联合盐酸利托君治疗)与对照组(单纯盐酸利托君保守安胎治疗)159例,对比分析两组的分娩孕周、孕周延长时间,随访新生儿Apgar评分、出生体质量,新生儿不良结局.结果 经过统计分析,研究组的分娩孕周及其延长时间高于对照组,差异有统计学意义(P＜0.05);研究组的新生儿Apgar评分、出生体质量均高于对照组,差异有统计学意义(P＜0.05);研究组治疗后新生儿不良结局6例(3.03％),低于对照组[21例(13.21％)],差异有统计学意义(P＜0.05).结论 紧急宫颈环扎术联合盐酸利托君用于宫颈机能不全性晚期流产患者,可以改善新生儿Apgar评分,提升体质量,降低新生儿不良结局,安全性更高,可延长孕周时间.
Abstract
Objective To explore the effect of emergency cervical cerclage and Lidocaine Hydrochloride combined treatment on patients with advanced miscarriage due to cervical insufficiency.Methods 357 patients with advanced miscarriage due to cervical insufficiency were selected from the Obstetrics and Gynecology department of our hospital from December 2019 to December 2023.They were divided into the study group of 198 cases(emergency cervical cerclage combined with rituximab hydrochloride treatment)and the control group of 159 cases(conservative treatment with rituximab hydrochloride)based on whether surgery was performed or not.The gestational age and duration of delivery were compared and analyzed,and the Apgar score,birth weight,and adverse outcomes of newborns were followed up.Results After statistical analysis,the gestational age and duration of delivery in the study group were higher than those in the control group,and the difference was statistically significant(P＜0.05);After statistical analysis,the Apgar score and birth weight of newborns in the study group were higher than those in the control group,and the difference was statistically significant(P＜0.05);After treatment,6 cases(3.03％)of neonatal adverse outcomes in the study group were lower than that of the control group of 21 cases(13.21％),and the difference was statistically significant(P＜0.05).Conclusion The combination of emergency cervical cerclage and Lidocaine Hydrochloride treatment can improve the Apgar score of newborns,enhance body mass,reduce adverse neonatal outcomes,increase safety,and prolong gestational weeks in patients with late stage miscarriage due to cervical insufficiency.
