摘要
目的 分析急性胰腺炎患者早期肠内营养支持过程中使用谷氨酰胺联合地衣芽孢杆菌的治疗效果及对营养水平的影响.方法 回顾性分析福州市第二总医院2020年3月至2023年6月收治的共72例急性胰腺炎患者的临床资料,其中对照组36例(采用谷氨酰胺肠溶胶囊治疗)、观察组36例(采用谷氨酰胺联合地衣芽孢杆菌治疗).对比两组患者住院期间病情改善时间,包括腹痛缓解时间、住院时长、血淀粉酶恢复时间;营养水平变化情况,包括白蛋白、前白蛋白、血红蛋白;炎症指标变化情况,包括白细胞计数、C反应蛋白、降钙素原,以及治疗后72 h临床疗效评价等.结果 通过对比两组急性胰腺炎患者临床症状的变化可发现,观察组患者临床症状消失时间、营养指标以及血氧炎症因子等数据整体优于对照组,差异有统计学意义(P＜0.05);观察组急性胰腺炎患者的治疗总有效率高于对照组,差异有统计学意义(P＜0.05).结论 在急性胰腺炎患者早期肠内营养治疗中,应用谷氨酰胺联合地衣芽孢杆菌的治疗效果显著,值得临床进一步推广与研究.
Abstract
Objective To analyze the therapeutic effect of glutamine combined with Bacillus subtilis in the early enteral nutrition support process of patients with acute pancreatitis and its impact on nutrition levels.Meth-ods A retrospective analysis of the clinical data of 72 patients with acute pancreatitis admitted to our hospital from March 2020 to June 2023 was conducted.Among them,there were 36 cases in the control group(treated with glutamine enteric-coated capsules)and 36 cases in the observation group(treated with glutamine combined with Bacillus subtilis).The improvement in the condition during hospitalization[relief time of abdominal pain,length of hospital stay,amylase recovery time],changes in nutritional levels[albumin,prealbumin,hemoglobin],changes in inflammatory markers[white blood cell count,C-reactive protein,procalcitonin],and clinical efficacy evalua-tion after 72 hours of treatment were compared between the two groups.Results By analyzing the changes in clinical symptoms of the two groups of patients with acute pancreatitis,it was found that the observation group had overall better disappearance time of clinical symptoms,nutritional indicators,blood oxygen inflammatory factors,etc.,compared withthose of the control group,and the differences were statistically significant(P＜0.05);the total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion In the early enteral nutrition treatment of patients with acute pan-creatitis,the therapeutic effect of using glutamine combined with Bacillus subtilis is significant,which is worthy of further clinical promotion and research.
