摘要
目的 探究支原体肺炎患儿中性粒细胞-淋巴细胞比值(NLR)、降钙素原(PCT)、D-二聚体(D-D)表达水平,以及疾病预后的评估作用.方法 以2021年9月至2023年9月邵武市立医院确诊支原体肺炎的81个患儿为研究对象,以预后状况作为分组依据,分为预后良好组(66例)、预后不良组(15例),收集患儿临床资料,检测两组患儿血清NLR、PCT、D-D水平.分析NLR、PCT、D-D水平对支原体肺炎患儿预后的预测价值.结果 单因素分析结果显示,白细胞计数(WBC)、血小板计数(PLT)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、NLR、PCT、D-D水平差异具有统计学意义(P<0.05).Logistic多因素回归分析显示,TNF-α、NLR、PCT、D-D水平是影响支原体肺炎患儿预后的危险因素(P<0.05).ROC曲线分析显示NLR、PCT、D-D三者联合检测对支原体肺炎患儿预后有一定的预测作用,且联合预测AUC(0.974)高于单个变量的AUC,提示联合预测支原体肺炎患儿预后的价值和准确度更高.结论 NLR、PCT、D-D是支原体肺炎患儿预后的危险因素,且NLR、PCT、D-D联合预测支原体肺炎患儿预后的准确度高于单个变量,诊断价值更高.
Abstract
Objective To investigate the expression levels of neutrophil to lymphocyte ratio(NLR),pro-calcitonin(PCT),and D-dimer(D-D)in children with Mycoplasma pneumonia,as well as their role in evaluating disease prognosis.Methods Data analysis was conducted on 81 confirmed cases of Mycoplasma pneumonia in Shaowu Municipal Hospitalfrom September 2021 to September 2023.Referring to the prognosis,66 cases with good prognosis and 15 cases with poor prognosis were divided into two groups.Clinical data of the patients were collected,and the NLR,PCT,and D-D levels of the two groups of patients were tested.We analyzed the predictive value of NLR,PCT,and D-D on the prognosis of children with Mycoplasma pneumonia.Results According to the results of single factor analysis,the difference of WBC,PLT,IL-6,TNF-α,CRP,NLR,PCT,and D-D index val-ues of the two groups were statistically significant(P<0.05).According to the results of logistic multiple factor re-gression analysis,TNF-α,NLR,PCT,and D-D are risk factors affecting the prognosis of children with Mycoplasma pneumonia(P<0.05).ROC curve analysis shows that the combination of NLR,PCT,and D-D has a certain predic-tive effect on the prognosis of children with Mycoplasma pneumonia,and the combined prediction AUC(0.974)is higher than the AUC of all single predictions,indicating that the combined prediction has higher value and accuracy in predicting the prognosis of children with Mycoplasma pneumonia.Conclusion NLR,PCT,and D-D are risk factors for the prognosis of children with Mycoplasma pneumonia,and the combined prediction of NLR,PCT,and D-D has higher accuracy in predicting the prognosis of children with Mycoplasma pneumonia than that of all single predictions,indicating higher diagnostic value.
