药房中药饮片处方用药在临床中的应用分析

目的通过分析晋江市医院药房的中药饮片的2100张处方,提高中药饮片处方质量,强化中药饮片使用的合理性.方法选取2023年1月至2023年12月晋江市医院药房的中药饮片处方,随机抽取2100张,分析其基本使用情况及不合格中药饮片处方原因.结果排名前三的疾病为消化系统(631张,30.05％)、肿瘤类(447张,21.29％)和心血管系统疾病(299张,14.24％);患者年龄分布最多为51～65岁(644张,30.67％),其次为66～80岁(436张,20.76％);女性占58.90％,高于男性的41.10％.使用11～20味中药的处方697张,占比最高(33.19％);其次为使用2～10味中药的处方,为624张(29.71％);使用单味中药的处方有51张,占比最低(2.43％).2100张处方中,不合格的处方有386张,占所抽查处方的18.38％.其中,临床诊断不规范140张(6.67％);书写不规范219张(10.40％);使用方法不当27张(1.29％).使用中药饮片中存在单味中药超量及使用毒性药物的不合理性.其中,用药超量处方有186张(8.86％),使用含毒饮片处方有363张(17.29％),毒性药物超正常剂量处方有154张(7.33％).结论我院药房中药饮片处方使用量较大,药物整体使用合理,但也存在一些药物不合理使用的情况,临床应进一步加强管理,保障中药饮片处方的规范化.

Abstract

Objective To analyze and comment on 2100 prescriptions of Chinese herbal decoction pieces in pharmacy of our hospital,improve the quality of prescriptions of Chinese herbal decoction pieces in our hospi-tal,and strengthen the rationality of the use of Chinese herbal decoction pieces.Methods 2100 Chinese herbal medicine prescriptions from the hospital pharmacy in Jinjiang City from January 2023 to December 2023 were randomly selected,and their basic usage and reasons for unqualified Chinese herbal medicine prescriptions were analyzed.Results Among the 2100 prescriptions,the top three diseases treated were digestive system(631 prescriptions,30.05％),tumor(447 prescriptions,21.29％)and cardiovascular system(299 prescriptions,14.24％).The age of most patients was 51-65 years old(644 prescriptions,30.67％),followed by 66-80 years old(436 pre-scriptions,20.76％).Female patients accounted for 58.90％,which was higher than male patients(41.10％).There were 697 prescriptions with 11-20 Chinese herbals,accounting for the highest proportion(33.19％),followed by 624 prescriptions(29.71％)with2～10 Chinese herbals,and 51 prescriptions with single Chinese herbal,accounting for the lowest proportion(2.43％).Among the 2,100 prescriptions,386 were unqualified,accounting for 18.38％of the prescriptions sampled.Among them,140(6.67％)prescriptions were not standardized in clinical diagnosis,219(10.40％)prescriptions were not in standard writing,and 27(1.29％)prescriptions were used inappropriately.There is an overdose of single Chinese herbaland the irrationality of using toxic herbals in the use of Chinese herbal decoc-tion pieces in our hospital.Among them,186(8.86％)prescriptions were prescribed for overdose,363(17.29％)pre-scriptions were prescribed for using poisoned decoction pieces,and 154(7.33％)prescriptions were prescribed for overdose of toxic drugs.Conclusion The prescription of Chinese medicine decoction pieces in the pharmacy of our hospital is large,the overall use of drugs is reasonable,and there are also some irrational use of drugs,so the clinical management should be further strengthened to ensure the standardization of the prescription of Chi-nese medicine decoction pieces.
