摘要

目的 探讨脐血外泌体在新生儿早发型脓毒症中的预测作用和早期诊断价值.方法 选取2022年1月至2024年4月天台县人民医院接收的50例新生儿早发型脓毒症患儿为观察组,选取同期出生的50例健康新生儿为对照组,利用酶联免疫吸附测定法(Enzyme-Linked Immuno Sorbent Assay,ELISA)测定脐血外泌体蛋白(SOCS-1、CD9、CD63、CD81、Annexin V)的含量,并评估其在新生儿脓毒症早期诊断中的灵敏度、特异性和准确度.结果 观察组患儿脐血外泌体蛋白SOCS-1、CD9、CD63、CD81的含量显著高于对照组,Annexin V的含量显著低于对照组,差异均有统计学意义(P<0.001).SOCS-1、CD9、CD63、CD81、Annexin V联合检测对新生儿早发型脓毒症诊断的准确度为90.05%,灵敏度为87.88%,特异性为91.22%.结论 脐带血外泌体SOCS-1、CD9、CD63、CD81、Annexin V蛋白联合检测在新生儿早发型脓毒症中具有良好的诊断价值.

Abstract

Objective To investigate the predictive role and early diagnostic value of umbilical cord blood exosomes in early-onset neonatal sepsis.Methods 50 neonates with early-onset sepsis admitted to Tiantai Peo-ple's Hospital from January 2022 to April 2024 were selected as the observation group,and 50 healthy neonates born in our hospital during the same period were selected as the control group.ELISA was used to determine the content of umbilical cord blood exosome proteins(SOCS-1,CD9,CD63,CD81,Annexin V),and evaluate their sensitivity,specificity,and accuracy in the early diagnosis of neonatal sepsis.Results The content of umbilical cord blood exosome proteins SOCS-1,CD9,CD63,CD81 in the observation group was significantly higher than that in the control group,while the content of Annexin V was significantly lower than that in the control group,and the differences were statistically significant(P<0.001);The accuracy of diagnosing early-onset neonatal sepsis with a combination of SOCS-1,CD9,CD63,CD81,and Annexin was 90.05%,with a sensitivity of 87.88%and a specificity of 91.22%.Conclusion Detection of umbilical cord blood exosome proteins SOCS-1,CD9,CD63,CD81,and Annexin has good diagnostic value in early-onset neonatal sepsis.
