摘要
目的 分析舌下含服卡托普利联合尼卡地平对高血压亚急症治疗的临床疗效影响.方法 选取2020年10月至2023年9月商河县人民医院收治的150例高血压亚急症患者,采用随机数字表法分为观察组与对照组,每组各75例,对照组采取注射尼卡地平及饮食管理治疗,观察组在对照组治疗基础上舌下含服卡托普利治疗,比较两组临床疗效,对两组患者治疗前后血管紧张素Ⅱ(Angiotensin Ⅱ,Ang Ⅱ)、醛固酮(aldosterone,ALD)水平、肾素(Renin)、促肾上腺皮质激素(adrenocorticotrophic hormone,ACTH)、皮质醇(Cortisol,Cor)、血压水平(舒张压、收缩压)进行对比分析.结果 治疗后观察组比对照组临床疗效更好,差异具有统计学意义(P＜0.05);两组患者治疗前Ang Ⅱ、ALD水平、Renin、ACTH、Cor水平均无显著差异(P＞0.05),治疗后观察组Ang Ⅱ、ALD、Renin、ACTH、Cor水平较对照组降低更显著(P＜0.05).两组患者治疗前血压水平比较无显著差异(P＞0.05),治疗后观察组血压较对照组显著降低,差异具有统计学意义(P＜0.05).结论 舌下含服卡托普利联合尼卡地平可在治疗高血压亚急症中取得理想疗效,可稳定患者血压水平,降低Ang Ⅱ、ALD、Renin、ACTH、Cor水平.
Abstract
Objective To analyze the clinical efficacy of sublingual administration of captopril combined with nifedipine in the treatment of hypertensive urgency.Methods 150 patients with hypertensive urgency treat-ed at Shanghe County People's Hospital from October 2020 to September 2023 were selected as the study subjects.They were randomly divided into the observation group and the control group,with 75 cases in each group.The control group received treatment including injection nifedipine and dietary management.The observation group received sublingual administration of captopril in addition to the treatment given to the control group.Clinical effi-cacy was compared between the two groups.Levels of Ang Ⅱ,ALD,renin,adrenocorticotrophic hormone(ACTH),cortisol(Cor),and blood pressure(diastolic and systolic)were analyzed before and after treatment in both groups.Results After treatment,the observation group showed better clinical efficacy compared to the control group,with statistically significant differences(P＜0.05).There were no significant differences in levels of Ang Ⅱ,ALD,renin,ACTH,and Cor between the two groups before treatment(P＞0.05).After treatment,levels of Ang Ⅱ,ALD,renin,ACTH,and Cor decreased more significantly in the observation group compared to the control group(P＜0.05).There were no significant differences in blood pressure levels between the two groups before treatment(P＞0.05).But after treatment,the observation group showed a significantly greater reduction in blood pressure compared to the control group,with statistically significant differences(P＜0.05).Conclusion Sublingual administration of captopril combined with nifedipine achieves favorable efficacy in the treatment of hypertensive urgency,stabilizes blood pressure levels,and reduces levels of Ang Ⅱ,ALD,renin,ACTH,and Cor in patients.
