摘要

目的 探讨重复经颅磁刺激(Repetitive Transcranial Magnetic Stimulation,rTMS)联合悬吊训练(Sling Exercise Therapy,SET)对脑卒中偏瘫患者平衡功能的影响.方法 选取2022年5月至2023年8月蚌埠医科大学第一附属医院收治的90例脑卒中恢复期伴平衡功能障碍患者,按照随机数字表法分入常规组(n=30)、磁刺激组(n=30)和联合组(n=30).常规组仅接受常规康复治疗,磁刺激组在常规康复治疗基础上增加健侧1Hz重复经颅磁刺激治疗;联合组在常规康复训练基础上增加健侧1Hz重复经颅磁刺激联合悬吊训练治疗.治疗前和治疗4周后采用Berg平衡量表(BBS)、改良Barthel指数(MBI)以及采集竖脊肌表面肌电平均肌电值(AEMG)和中位频率值(MF)对患者进行评定.结果 治疗后,患者BBS评分、MBI评分、AEMG值和MF值均较治疗前有明显改善,差异具有统计学意义(P<0.05),其中,联合组治疗后的BBS评分、MBI评分、AEMG值和MF值高于常规组和磁刺激组(P<0.05),磁刺激组高于常规组(P<0.05).结论 rTMS联合SET能够改善脑卒中患者的竖脊肌AEMG、MF值及BBS和MBI评分,提高脑卒中患者躯干肌肉运动功能,从而提高平衡功能以及日常生活活动能力,值得在临床推广应用.

Abstract

Objective Exploring the effect of repetitive transcranial magnetic stimulation(rTMS)combined with suspension training(SET)on balance function in stroke patients with hemiplegia.Methods According to the random number table method,participants were divided into a conventional group(n=30),a magnetic stimula-tion group(n=30),and a combination group(n=30).The conventional group only received conventional rehabilita-tion treatment,while the magnetic stimulation group received additional l Hz repetitive transcranial magnetic stim-ulation treatment on the healthy side in addition to conventional rehabilitation treatment;The joint group received additional 1Hz repetitive transcranial magnetic stimulation combined with suspension training on the healthy side on the basis of routine rehabilitation training.Before and after 4 weeks of treatment,patients were evaluated using the Berg Balance Scale(BBS),Modified Barthel Index(MBI),and average surface electromyography(AEMG)and median frequency(MF)of the erector spinae muscle.Results After treatment,the patient's BBS score,MBI score,AEMG value,and MF value all showed significant improvement compared to before treatment,and the dif-ferences were statistically significant(P<0.05).Among them,the BBS score,MBI score,AEMG value,and MF val-ue of the combination group after treatment were higher than those of the conventional group and the magnetic stimulation group(P<0.05).The magnetic stimulation group was higher than the conventional group(P<0.05).Conclusion rTMS combined with SET can improve the erector spinae muscle AEMG in stroke patients The MF value and BBS and MBI scores can improve the trunk muscle movement function of stroke patients,thereby enhancing balance function and daily living activity ability,and are worthy of clinical promotion and application.
