摘要

目的 探索盆底肌肉锻炼结合电刺激生物反馈的治疗手段在盆底功能障碍患者中的应用价值.方法 回顾性选取2022年1月至2023年6月于六枝特区人民医院就诊的120例盆底功能障碍患者资料,根据治疗方案的差异分为观察组(盆底肌肉锻炼+电刺激生物反馈,n=60)和对照组(盆底肌肉锻炼,n=60).比较两组患者Ⅰ、Ⅱ类肌纤维肌力、疲劳度,血清盆底肌肉松弛相关指标.结果 治疗前,两组患者Ⅰ类肌纤维肌力和疲劳度均无显著差异(P>0.05).治疗后,两组患者Ⅰ、Ⅱ类肌纤维肌力均提高,疲劳度均降低;观察组患者治疗后1个月、3个月、6个月Ⅰ、Ⅱ类肌纤维的Ⅰ~Ⅲ级肌力占比低于对照组,Ⅳ~Ⅴ级肌力占比高于对照组(P<0.05);且观察组患者治疗后1个月、3个月、6个月Ⅰ、Ⅱ类肌纤维疲劳度均显著低于对照组(P<0.05).治疗前,两组患者血清盆底肌肉松弛相关指标松弛素(Relaxin,RLX)、一氧化氮(NO)、内皮素(Endothelin-1,ET-1)均无显著差异(P>0.05).治疗后,两组患者RLX和ET-1水平均降低,NO水平升高;观察组患者治疗后1个月、3个月、6个月的RLX和ET-1水平显著低于对照组(P<0.05),NO水平显著高于对照组(P<0.05).结论 盆底肌肉锻炼联合电刺激生物反馈的治疗手段可有效改善盆底功能障碍患者的盆底肌力、疲劳度和血清盆底肌肉松弛相关指标.

Abstract

Objective To explore the therapeutic value of combining pelvic floor muscle training with elec-trical stimulation biofeedback in patients with pelvic floor dysfunction.Methods A retrospective analysis was conducted on the clinical data of 120 patients with pelvic floor dysfunction who were treated at Liuzhi Special Dis-trict People's Hospital between January 2022 and June 2023.Based on the treatment methods,patients were divided into the observation group(pelvic floor muscle training combined with electrical stimulation biofeedback,n=60)and the control group(pelvic floor muscle training alone,n=60).The muscle strength and fatigue levels of type Ⅰ and Ⅱmuscle fibers,as well as serum markers related to pelvic floor muscle relaxation,were compared between the two groups.Results Before treatment,there were no significant differences in type Ⅰ and Ⅱ muscle fiber strength and fatigue levels between the two groups(P>0.05).After treatment,both groups showed improved type Ⅰ and Ⅱ muscle fiber strength and reduced fatigue levels.At 1 month,3 months,and 6 months after treatment,the observation group had a lower proportion of type Ⅰ and Ⅱ muscle fibers with grade Ⅰ-Ⅲ strength and a higher proportion with grade Ⅳ-Ⅴ strength compared to the control group(P<0.05).Additionally,fatigue levels of type Ⅰ and Ⅱ muscle fibers in the observation group were significantly lower than those in the control group at the same time points(P<0.05).Before treatment,there were no significant differences between the two groups in serum markers related to pelvic floor muscle relaxation,including relaxin(RLX),nitric oxide(NO),and endothelin-1(ET-1)(P>0.05).After treatment,RLX and ET-1 levels decreased,while NO levels in-creased in both groups.However,at 1 month,3 months,and 6 months post-treatment,the observation group had significantly lower RLX and ET-1 levels and higher NO levels compared to the control group(P<0.05).Conclusion The combination of pelvic floor muscle training and electrical stimulation biofeedback effectively improves muscle strength,reduces fatigue,and modulates serum markers related to pelvic floor muscle relaxation in patients with pelvic floor dysfunction.
