摘要
目的 探讨溶血三项、网织红细胞与血清总胆红素联合在新生儿溶血病(HDN)中的诊断价值.方法 回顾性选取2021年3月至2024年3月于石狮市妇幼保健院就诊的80例疑似新生儿溶血病患儿为研究对象.根据金标准检测结果分为溶血病组(n=65)和非溶血病组(n=15).分析不同指标检测对新生儿溶血的诊断效能.结果 80例疑似患儿,溶血三项示61例(76.25%)为新生儿溶血病,19例(23.75%)为非新生儿溶血病.溶血病组患儿网织红细胞和血清总胆红素水平均显著高于非溶血病组(P<0.001).三者联合诊断的灵敏度显著高于单独网织红细胞(P=0.015)、血清总胆红素(P=0.048)诊断的灵敏度;三者联合诊断的特异度显著高于单独网织红细胞(P=0.013)、血清总胆红素(P=0.031)诊断的特异度;三者联合诊断的准确性显著高于单独溶血三项(P=0.043)、网织红细胞(P=0.001)、血清总胆红素(P=0.005)诊断的准确性.结论 三者联合可显著提高HDN的早期诊断效能.
Abstract
Objective To explore the diagnostic value of the combination of hemolytic markers,reticulocytes,and serum total bilirubin in hemolytic disease of the newborn (HDN). Methods 80 children suspected of neonatal hemolysis who visited Shishi Maternal and Child Health Hospital from March 2021 to March 2024 were selected as the research subjects. According to the gold standard test results,they were divided into hemolytic disease group (n=65) and non hemolytic disease group (n=15). Analyze the diagnostic efficacy of different indicator tests for neonatal hemolysis. Results Out of 80 suspected cases,61 (76.25%) were diagnosed with hemolytic disease of the newborn,while 19 (23.75%) were diagnosed with non hemolytic disease of the newborn. The levels of reticulocytes and serum total bilirubin in the hemolytic disease group were significantly higher than those in the non hemolytic disease group (P<0.001). The sensitivity of the combined diagnosis of the three is significantly higher than that of the diagnosis of individual reticulocytes (P=0.015) and serum total bilirubin (P=0.048);The specificity of the combined diagnosis of the three is significantly higher than that of the diagnosis of individual reticulocytes (P=0.013) and serum total biliru-bin (P=0.031);The accuracy of the combined diagnosis of the three factors is significantly higher than that of hemoly-sis alone (P=0.043),reticulocytes (P=0.001),and serum total bilirubin (P=0.005). Conclusion The combination of the three can significantly improve the early diagnostic efficiency of HDN.
