摘要
目的 探索磁共振检查(Magnetic Resonance,MR)对不同类型急性肩关节损伤的诊断价值.方法 回顾性分析2022年1月至2023年12月在兴化市中医院就诊的80例急性肩关节损伤患者的临床资料.以关节镜检查为金标准,分析MR检查对急性肩关节损伤诊断的敏感度、特异度和准确性,并进行Kappa一致性检验.结果 80例患者中,肩袖损伤MR诊断的敏感度为91.53％(54/59)、特异度为80.95％(17/21)、准确性为88.75％(71/80);MR诊断为肩袖损伤的一致性为(Kappa=0.714).肩关节撞击综合征MR诊断的敏感度为88.24％(15/17)、特异度为95.24％(60/63)、准确性为93.75％(75/80);MR诊断为肩关节撞击综合征的一致性为(Kappa=0.817).盂唇撕裂MR诊断的敏感度为75.00％(3/4)、特异度为98.68％(75/76)、准确性为97.50％(78/80);MR诊断为盂唇撕裂的一致性为(Kappa=0.737).结论 MR检查对肩袖损伤、肩关节撞击综合征、盂唇撕裂诊断的敏感度、特异度和准确性均较高,诊断效果与关节镜相符,临床价值显著.
Abstract
Objective To explore the diagnostic value of MR imaging for different types of acute shoulder injuries. Methods A retrospective analysis was conducted on the clinical data of 80 patients with acute shoul-der injuries who were treated at Xinghua City Hospital of Traditional Chinese Medicine from January 2022 to De-cember 2023. Using arthroscopy as the gold standard,the sensitivity,specificity,and accuracy of MR imaging in diagnosing acute shoulder injuries were analyzed,and Kappa consistency tests were performed. Results Among the 80 patients,the sensitivity,specificity,and accuracy of MR imaging for diagnosing rotator cuff injuries were 91.53％ (54/59),80.95％ (17/21),and 88.75％ (71/80),respectively. The consistency of MR imaging diagnosis for rotator cuff injuries was Kappa=0.714. For shoulder impingement syndrome,the sensitivity,specificity,and accura-cy were 88.24％ (15/17),95.24％ (60/63),and 93.75％ (75/80),respectively,with a consistency of Kappa=0.817. For labral tears,the sensitivity,specificity,and accuracy were 75.00％ (3/4),98.68％ (75/76),and 97.50％ (78/80),respectively,with a consistency of Kappa=0.737. Conclusion MR imaging demonstrates high sensitivity,speci-ficity,and accuracy in diagnosing rotator cuff injuries,shoulder impingement syndrome,and labral tears,with diag-nostic performance comparable to arthroscopy,thus holding significant clinical value.
