摘要
目的 分析对于高龄晚期前列腺癌并急性尿潴留患者采取罗伊—纽曼综合模式护理干预的临床价值.方法 选取2018年4月至2023年4月肇庆市第一人民医院泌尿外科收治的135例高龄晚期前列腺癌伴急性尿潴留患者,采用随机数字表法分为观察组(罗伊—纽曼综合模式护理组,68例)和对照组(常规护理组,67例).对比两组患者的焦虑、抑郁心理,希望水平、应对方式和生活质量.结果 干预前,两组患者焦虑自评量表(Self-rating Anxiety Scale,SAS)评分、抑郁自评量表(Self-rating Depression Scale,SDS)评分、Herth希望量表(Herth Hope Indes,HHI)评分、医学应对问卷(Medical Coping Modes Questionnaire,MCMQ)评分、回避评分、屈服评分、生活质量问卷(Quality Of Life,QOL)评分均无显著差异(P>0.05).干预后,两组患者SAS评分、SDS评分、回避评分、屈服评分均降低,而HHI评分、MCMQ评分、QOL评分均提高;且干预后观察组患者的SAS评分、SDS评分、回避评分、屈服评分显著低于对照组(P<0.001),HHI评分、MCMQ评分、QOL评分显著高于对照组(P<0.001).结论 对于高龄晚期前列腺癌并急性尿潴留患者,罗伊—纽曼综合模式护理干预有利于缓解患者焦虑、抑郁的不良心理,改善应对方式,提高患者希望水平和生活质量.
Abstract
Objective To analyze the clinical value of applying the Roy-Adaptation Model-based Newman Systems Model nursing intervention for elderly patients with advanced prostate cancer complicated by acute urinary retention. Methods A total of 135 elderly patients with advanced prostate cancer and acute urinary retention,admitted to the Department of Urology at Zhaoqing First People's Hospital from April 2018 to April 2023,were se-lected for this study. The patients were randomly assigned to the Roy-Adaptation Model-based Newman Systems Model nursing group (observation group,n=68) or the conventional care group (control group,n=67). Anxiety,de-pression,hope levels,coping strategies,and quality of life were compared between the two groups. Results Dur-ing the intervention period,there were no significant differences between the two groups in SAS scores,SDS scores,HHI scores,MCMQ confrontation scores,avoidance scores,resignation scores,or QOL scores (P>0.05). After the intervention,SAS scores,SDS scores,avoidance scores,and resignation scores decreased in both groups,while HHI scores,confrontation scores,and QOL scores increased. Additionally,post-intervention,the observation group had significantly lower SAS scores,SDS scores,avoidance scores,and resignation scores compared to the control group (P<0.001),while HHI scores,confrontation scores,and QOL scores were significantly higher in the observa-tion group than in the control group (P<0.001). Con-clusion For elderly patients with advanced prostate cancer and acute urinary retention,the Roy-Adaptation Model-based Newman Systems Model nursing inter-vention is effective in alleviating anxiety and depres-sion,improving coping strategies,and enhancing hope and quality of life.
