Research on Relevant Indicators for Early Diagnosis of Urinary Sepsis
Objective To explore the early diagnostic value of chitinase-3-like protein-1(CHI3L1)in urine for patients with urinary sepsis. Methods Clinical data of 50 patients with urinary sepsis admitted to our hospital from January 2023 to June 2023 were selected as the observation group. And clinical data of 50 patients with non urogenic sepsis who were treated during the same period were selected as the control group. The lev-els of serum procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),and CHI3L1 in urine between two groups were compared.Pearson analysis was used to analyze the correlation between CHI3L1 in urine and serum PCT,CRP,and WBC.Receiver operating curve(ROC)was used to analyze the diagnostic value of CHI3L1 in urine for urinary sepsis. Results The levels of serum PCT,CRP,WBC,and CHI3L1 in urine in the observation group were(4.61±0.64)μg/L,(50.76±5.43)mg/L,(16.82±2.59)×109/L,and(194.98±33.65)pg/mg,which were significantly higher than those in the control group(3.36±0.40)μg/L,(40.31±4.49)mg/L,(13.65±1.74)×109/L,and(102.93±28.34)pg/mg,with statistical significance(P<0.05).According to Pearson correlation analysis,CHI3L1 in urine was positively correlated with PCT,CRP,and WBC(r=0.615,0.533,0.432,P<0.05).ROC analysis showed that the area under the curve(AUC)of CHI3L1 in urine for diagnosing urinary sepsis was 0.974,with a 95%CI of 0.946~1.000,the sensitivity was 96%,the specificity was 92%,and the cutoff value was 140.26 pg/mg.Conclusion The level of CHI3L1 in the urine of patients with urinary sepsis is significantly elevated, and CHI3L1 in the urine has high diagnostic value for urinary sepsis, which should be closely monitored in clinical practice.
