摘要
目的 探讨应用颅内动脉瘤介入栓塞术治疗高分级动脉瘤性蛛网膜下腔出血(aSAH)效果及对格拉斯哥预后评分(GOS评分)及免疫球蛋白水平的影响.方法 选取 2020 年 10 月至 2023 年 10 月收治的 60 例高分级aSAH患者,按照随机数字表法原则,分成实验组、对照组各 30 例.对照组患者应用显微镜下夹闭术治疗,实验组患者应用颅内动脉瘤介入栓塞术治疗.比较两组GOS评分、免疫球蛋白[IgA、IgG、IgM水平]及术后并发症发生情况.结果 实验组术后 1、2、3 个月GOS评分均高于对照组(P＜0.05);两组术后 1d免疫球蛋白水平均较术前时刻降低,实验组术后 1d的IgA、IgG、IgM均高于对照组(P＜0.05);实验组术后并发症总发生率低于对照组的(P＜0.05).结论 对于高分级aSAH患者,采取颅内动脉瘤介入栓塞术治疗可显著提高患者GOS评分,维持免疫球蛋白水平,同时术后并发症发生率低.
Abstract
Objective To investigate the effect of interventional embolization of intracranial aneurysms in the treatment of high-grade a-neurysmal subarachnoid hemorrhage(aSAH)and its effect on Glasgow prognosis score(GOS)and immunoglobulin level.Methods A total of 60 pa-tients with high-grade aSAH treated from October 2020 to October 2023 were selected and divided into the experimental group and the control group with 30 patients in each group according to the principle of random number table.The control group was treated with microscope occlusion,and the experimental group was treated with intracranial aneurysm interventional embolization.GOS score,immunoglobulin(IgA,IgG,IgM levels)and postop-erative complications were compared between the two groups.Results The GOS score of the experimental group was lower than that of the control group 1,2 and 3 months after operation(P<0.05).The levels of immunoglobulin in the experimental group were lower than those in the control group one day after surgery(P<0.05).The total incidence of postoperative complications in experimental group was lower than that in control group(P<0.05).Conclusion For patients with high-grade aSAH,interventional embolization of intracranial aneurysm can significantly improve the GOS score,main-tain the level of immunoglobulin,and reduce the incidence of postoperative complications,which is worthy of promotion.
